The Care Coordinator position with Golden Heart is a valuable position where we will train you to be a reliable contact for our caregivers who are serving our community through the quality care they provide. You will also be an important component to our clients in order for them to get the best care through your skills of assessing their needs and training our caregivers to meet those specific needs.

Here at Golden Heart we provide quality care to each individual client because we know that everyone has different needs and needs to be provided with individualized care throughout their time with Golden Heart. As our care coordinator we will provide you with the tools and resources to be the heart beat of our Golden Hearts.

Care Coordination for clients and employees
Startup new clients and assess their unique situation
Field oversight
Accompany clients on doctor's appointments
Communicate with the family
Develop professional relationships with caregivers
Present solutions to caregivers that the company provides
Introducing caregivers to clients and begin the care plan so expectations are met and exceeded
Visiting clients that Golden Heart serves to ensure quality care is being given by our caregiversTrain and get to know the caregivers and clients
Review care plans and assess who best fits this job
Promote quality caregivers to new clients to give peace of mind to clients that are new to home care

Job Requirements

• Type minimum of 40 words a minute• Must be computer literate• Stellar written and verbal communication skills• Efficient under pressure• Healthcare experience or knowledge a plus• Must have experience making outbound calls• Must be reliable• Must have reliable vehicle• Sales experience would be helpful in this role• Must be comfortable and pleasant on the phone• Must be comfortable handling complex scheduling demands and managing multiple simultaneous client and field staff needs• Must be reliable with a good work ethic• Must be attentive and approachable• Must be proactive, astute, and quick thinking• Must be self-motivated and organized• Perform clerical duties• Maintain client confidentially• Demonstrate the ability to balance several task
