YOUR DUTIES AT A GLANCE:
In this position, you will assist case managers in the delivery of long-term care to our consumers. This position will assist in the coordination of Home Care Workers. You may make home visits in order to assure the appropriate continuity and effectiveness of developed service/care plans initiated by the case manager. Case aides may be used to recognize and report changes in functional/health status to the case manager. Case aides will be knowledgeable of issues representing potential abuse/abuse and will follow mandatory reporting policy.

WORKING CONDITIONS
Physical exertion such as climbing stairs, walking on uneven terrain and transporting a laptop. General office environment. Must travel to individuals' homes, hospitals, nursing homes, and other provider settings as well as other local offices and Salem. May encounter environmental hazards including unsanitary conditions. May come in contact with people who present as irrational, hostile,
or angry who may become assaultive verbally, physically or with a weapon. Also, will come in contact with animals, some of which may be dangerous. Drive a variety of cars which may be unfamiliar, to remote places in a variety of terrain and possibly inclement weather. Exposed to a variety of contagious diseases such as staph infections, lice, MRSA, COVID-19, etc. Welcomes constructive feedback and suggestions in an effort to strengthen work performance. Maintain confidentiality in accordance with published guidelines. Efficiently manage workload in a constantly changing work environment as well as plan for and resolve complex problems.
Office environment with daily use of computer/laptop and associated local and network environment. Working conditions include in-office with a small percentage that can be done working remotely intermittently.
WHAT YOU NEED TO QUALIFY/MINIMUM QUALIFICATIONS(MQ’s):
One year of experience assisting professionals in one or more of the following specialized social program areas by:
  • Assisting professionals who provide social program services to clients who are economically disadvantaged
  • Assisting professionals who provide social program services to clients who are substance dependent
  • Assisting professionals who provide social program services to clients who are mentally or physically disabled
  • Assisting professionals who provide social program services to clients who have experienced child abuse and neglect
  • Assisting professionals who provide social program services to clients who are juvenile delinquent(s)

Any degree in a related mental health or human services program may substitute for six months of the required experience.
ESSENTIAL ATTRIBUTES:
  • Experience in a work environment where there is a requirement to maintain confidentiality of sensitive information including financial and private health information
  • Advanced computer skills including creating spreadsheets with tables, graphs and formulas in a business office setting
  • Work experience in an office setting organizing and managing multiple tasks with frequent interruptions
  • Experience working in an office setting in a team environment working together planning, goal setting and covering other worker's duties
