Case Management Specialist Job at St. Joseph's/Candler
The Case Management Specialist manages a caseload of elderly or disabled persons, through St. Joseph's/Candler SOURCE or the Independent Waiver Program, promoting health and functional status and continued residence in the community for all members.
Ability to work independently within assigned deadlines; strong organizational skills; knowledge of local, state and regional health and social services; strong skills in verbal and written communication (including coordination with co-workers, SOURCE members, and advocates, physicians, providers and community members). Professional and personal commitment to quality, advocacy, productivity, creativity and ethics.
Requirements
Bachelors in Sociology - Preferred
5 to 7 years' healthcare or human services for the elderly or disabled - Preferred
