We are looking for a qualified CASE MANAGER that would enjoy making a positive impact on young people's lives in Randolph County. This is a part-time, 24- hour per week position. Travel is required, with mileage reimbursement per our company policy.

Requirements

• High School diploma or equivalent accredited by the proper regional board, with one year of work experience directly related to job responsibilities; Bachelor’s degree preferred
• Obtain the case management certification through the Alabama Department of Mental Health (ADMH) prior to completion of the probationary period, and prior to billing case management services
• Secure CPR and First Aide certification within thirty (30) days of the date of hire and maintain
• Valid Alabama driver’s license, proof of personal vehicle insurance, acceptable driving record and access to personal transportation to meet with clients in a variety of locations

Job Responsibilities / Essential Functions

• Provide case management services and assist in the intake of new clients
• Complete assessment tools, establish a case management file, and service plan
• Willing to work with families and clients during non-traditional hours in various settings such as offices, schools, and homes
• Actively participate in the treatment team case review, consultation, and continuing care planning activities
• Communicate with referral sources and continuing care placement resources for appointment scheduling, transportation arrangements, and other services to maximize the client’s opportunity for treatment success
• Maintain cooperative relationships with the treatment community, probation departments, and other necessary and authorized agencies
• Ensure content of client files is in full compliance with The Bridge Policies / Procedures, Medicaid, contracting agency standards/regulations
• Complete all required documentation in the electronic health record
• Reconcile service notes with billing records and enter billing data in the appropriate billing submission system
