Eliot’s Certified Community Behavioral Health Clinics (CCBHC) in Everett and Lynn are searching for case managers to provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragement. The goal of Eliot’s CCBHC is to expand the impact of evidence-based treatments for underserved populations in the Everett and Lynn areas.

The ideal candidate will be bilingual, will be able to establish effective working relationships with clients, and will utilize solution based interventions which best address specific problem area as identified in the client’s treatment plan. Eliot’s CCBHC is a multidisciplinary team including psychiatrists, nurse practitioners, nurses, case managers, triage specialists, recovery coaches, and peer specialists. The team administers a full range of clinical and support services, resulting in a comprehensive collaborative approach to treatment.

Responsibilities:

  • Provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragement.
  • Participate in daily Team Huddles and Team Meetings.
  • Assist the team and individuals in providing on-going assessment of symptoms and responses to staff interventions.
  • Participate in the development of treatment plans for each individual in which you are a member of their treatment team.
  • Ensure that each individual is actively involved in their treatment planning process and their preferences are embraced.
  • Ensure that documentation is written is measurable, observable, objective and utilizes clinical terms.
  • Provide interventions to individuals as stated by their current treatment plan or in response to the daily needs of individuals per supervisory direction and/or through the Team Huddle process.
  • Provide case management supports to individuals including: managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.

Qualifications:

  • Must have a bachelor's degree in psychology, social work or related field and three years experience.
  • Must have a strong commitment to recovery oriented services.
  • Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses and respect for their preferences for treatment.
  • Must have ability to provide flexible services that are community based and focused on team work.
  • Demonstrates sound judgment and effective, solution focused, problem solving skills.
  • Ability to work effectively with diverse populations and cultural groups.

Eliot offers a competitive benefits packages including: up to $3000 annually for professional development reimbursement, great health insurance of which Eliot pays 75%, three weeks vacation in the first year, 12 paid holidays off, and sick time. Pay differential for bilingual candidates.

Education
