Overview:
Kare-In-Home seeks an experienced CNA (must be licensed) to join our t eam in Saraland, AL serving Clarke and Washington Counties Full-Time. Areas served will include: Washington County:Leroy, Mcintosh, Chatom, Millry, Hobson, Frankville, Copeland, Fruitdale, Vinegar Bend, Deer Park , and Tibbie . Clarke County:Jackson, Grove Hill, Thomasville, and Gainestown.

Hospice aide services will include personal care services provided by trained hospice aides and non-skilled services provided by homemakers.

Hospice aide services may be included for routine care on an intermittent basis when personal care is needed. Services provided by an aide could include: bathing, shampoo, range of motion activities, dry dressing changes, assisting the patient to the bathroom, use of commode or bed pan, assisting with meal preparation, feeding the patient, light housekeeping and/or linen changes. These activities in accordance with the plan of care and physician's (or other authorized independent practitioner’s) orders will be assigned and supervised by a hospice registered nurse.

Homemaker services may be included for routine care on an intermittent basis when nonhands-on care is required. These types of services could include: housekeeping, family/caregiver respite, and meal preparation. Specifically, these activities would not be related to personal care; however, they would assist the primary family/caregiver to keep the patient at home.
Responsibilities:
  • The need for hospice aide services will be identified by the interdisciplinary group.
  • Arrangements for hospice aide service will be made by the case manager the Case Manager or Branch Director of appropriate designee. When needed, this service could be referred to a contract organization.
  • Hospice aide assignments, plan of care, and interventions will be documented and included in the patient's clinical record.
  • Hospice aide duties include:
    • Assisting with personal hygiene
    • Assisting with ambulation and exercise
    • Reporting changes in the patient’s condition and needs
    • Providing nutritional support
    • Other supportive tasks as assigned.
  • Hospice aides will document care provided in accordance with the hospice aide assignment.
  • Homemaker/chore services personnel will provide the patient and family/caregiver with environmental support under professional supervision.
  • Support and direct field staff and branch leadership with urgent patient related matters that can create stressful situations.
Qualifications:
Education/License: Highschool education or higher preferred for this position. Certified Nursing Assistance License required for state in which incumbent is working and assigned.

Experience:
  • Experience operating in a medical environment
  • Preferred is (3) three to (5) years of experience at minimum the documented (1) year of experience working as a certified nurse aide/homemaker in the last 24 months.
  • Has a working knowledge of hospice care and the different aspects that are involved in supporting both the field staff and leadership of the program.
Skills:
  • Be coachable with a goal to grow personally and professionally.
  • Embrace the hospice philosophy and the principles of death or dying.
  • Excellent communication skills.
  • Must be able to communicate with referral sources and all outside entities telephonically.

Transportation:
Will require some local day and/or overnight travel, approximately 90-100% of the time within the Hospice Division Service area..

Environmental/Working Conditions:
The incumbent must be able to move, stand and bend as necessary, to provide direct patient care at least 80-100% of the time. Able to position, move and lift medical supplies up to 50lbs. Ability to work in a variety of settings 90%-100% of the time including but not limited to a patient’s residence, wherever that may be, i.e. a nursing home, assisted living center or private residence. Ability to cope with a variety of residences where patients choose to live. Ability to cope with death and dying and the psychological effects.
