Summary:
Responsible for providing assistance to various members of clinical team. This incumbent is expected to:
  • Have a basic knowledge of practices and standards of the clinical team they are supporting
  • Assist clinical team in tracking, monitoring and completing clinical administrative duties
Essential Duties:
  • Administers, uploads, scores and enters Outcome Measure data into internal and external systems in accordance with program timelines.
  • Elevates client risk assessments in accordance with program trainings/protocols. Ensures immediate communication of client risks to treatment team, applicable supervisors, and emergency mental health providers in the community.
  • Reviews, completes, and monitors required program documentation by communicating with the team members and supervisors in a time-sensitive manner.
  • Assist the division with follow-up and monitoring/tracking of chart compliance using applicable reports. Ensures that client documents are scanned, uploaded into Welligent, and filed in client’s chart, per the chart model provided by QA as assurance of uniformity.
  • Collaborates/meets with program teams to ensure a consistent flow of information, program quality and professional delivery of services relative to each program.
  • Ensures various internal and external systems (i.e., SRTS, Call Center, HOPE line) are updated and maintained as assigned.
  • Prepares and completes adjunctive program and county reports as scheduled for the various programs in the Division; follows up as needed to ensure documents are complete.
  • Completes quarterly and periodic chart reviews for audit preparation and overall agency quality assurance protocols; maintains audit records and shares with program director/supervisor and agency’s quality assurance department.
  • Assists clinicians with case management responsibilities; meets with the clinician for assigned clients on a regular basis.
  • Maintains knowledge of program standards and policies as developed by Five Acres, Community Care Licensing, and other governing bodies; develops personal and professional knowledge by attending educational workshops; participating in in-service programs.
  • Exercises good judgment in the performance of duties and responsibilities.
Qualifications:
To perform effectively in this position, the incumbent must have:
  • High School Diploma or equivalent required; Bachelor’s degree, preferred
  • Must have a valid California Driver License, proof of automobile insurance and meet the Agency’s safe driver guidelines.
  • Availability of a personal car with appropriate insurance which employee is willing to use for all Agency business, if needed; adhering to federal motor vehicle safety standards when transporting children.
  • Effective communication skills both oral and written; bi-lingual skills in Spanish/English is preferred.
  • Ability to be flexible and work in a changing environment.
  • Ability to attend Day 1 & 2 of the Pro Act de-escalating system training.
  • Computer skills sufficient to meet production demands of the job including Microsoft Office and WELLIGENT software for program notes; and skill in maintaining accurate, detailed and complete records in a mental health environment.
  • Sensitivity to the different cultures represented among the clients, partners and staff
  • A basic understanding of age appropriate behavior for children/adolescents
Compensation Range: $17.23/hr - $24.12/hr
