White Horse Village is seeking to add a Clinical Manager to our Outpatient department! The Clinical Manager will oversee the Outpatient Department. The Clinical Manager will plan, develop, and provide the leadership for the directing, coordinating, and implementing of the services of Outpatient Department. The Clinical Manager will be a full time position.

Duties:

  • The Clinical Manager Oversees the Outpatient department with a team to provide healthcare services to residents in an active senior community.
  • The Clinical Manager provides home visits to determine health care needs post hospitalization and post discharge from the health care center.
  • The Clinical Manager provides complete home visits/assessments as needed.
  • The Clinical Manager evaluates home visits and assessments to determine cognitive/functional changes that indicate the need for supportive services or a level of care change.
  • The Clinical Manager responds to emergency calls and institutes appropriate emergency interventions as needed.
  • The Clinical Manager conducts both formal and informal educational programs
  • The Clinical Manager performs assessments based on residents’ needs and shares findings with the attending physician and team as appropriate.
  • The Clinical Manager plans and implements interventions to meet these needs, utilizing internal and external resources, as appropriate.
  • The Clinical Manager assists physicians in the screening of current and prospective residents.
  • The Clinical Manager oversees medication exchange program for residents in need of this service.
  • The Clinical Manager maintains resident healthcare records.
  • The Clinical Manager develops positive relationships with residents and family members/significant others.
  • The Clinical Manager performs quality improvement audits.
  • The Clinical Manager monitors and audit various areas for quality assurance
  • The Clinical Manager assists with Personal Care Home as needed for consultation and audits.
  • The Clinical Manager works some required weekends on call

Qualifications

  • Must hold a valid LPN or RN license
  • Minimum of 3 years in an outpatient/physicians office setting
  • Minimum of 3 years management experience
  • Personal Care experience a plus but not required
  • Excellent nursing judgement and assessment skills
