POSITION TITLE: Children’s Long Term Care Case Manager(CLTS)/Coordinated Services Team (CST)/Comprehensive Community Services (CCS) Coordinator

AGENCY UNIT: Adult Services

STARTING SALARY: $21.47 - $23.17 per hour

WORKDAY/WORKWEEK: 35 hours per week/4-day flexible work week

Job Summary

This position is responsible to provide services outlined in Wisconsin Statute 46.56. The main job duties will be managing the CST program, but will also include some case management for the CLTS position and some crossover with the CCS program. The CST position has three core areas: client services, program development, and marketing/education. The CCS position is responsible to provide services outlined in Chapter DHS Rule 36. CCS provides psychosocial rehabilitation services to consumers who have needs for ongoing, high or low intensity services resulting from mental health or substance use disorders. Position has extensive contact with the public, other professional service providers in the community and other employees in the department. Position functions are performed in an office, client home and community setting.

Qualifications

1) Minimum of a four-year degree from an accredited college in social work, sociology, psychology, criminal justice or other approved human services program.

2) Minimum of one year experience providing social work to children and families.

3) Preference will be given to individuals with social work experience, and those who are certified as a social worker in the State of Wisconsin or eligible for certification within one (1) year.

4) Valid Wisconsin driver’s license (available licensed driver), insurance and reliable transportation.

Duties and Responsibilities

  • The primary responsibility of the coordinator is to promote collaborative relationships between systems of care.
  • Work with the coordinating committee and service coordination agency to receive and review referrals.
  • Work with the coordinating committee and service coordination agency to assure provision of service coordination services for all groups of people working with the child and his or her family.
  • Guide the development of the coordinated service team working with the child and his or her family in order to ensure compliance with basic principles of the initiative core values.
  • Review plans of care, including crisis response plans, for consistency with the coordinated services team approach to providing services to a child and his or her family and core values.
  • Ensure provision of ongoing support and training that is related to the coordinated services team process for families, care/service coordinators, and providers and ensure orientation for coordinated services team members.
  • Establish and report monitoring and evaluation results.
  • Monitor, or ensure proper monitoring of targeted case management and in-home services provided under the Medical Assistance Program, including recordkeeping and billing processes.
  • Assist in the development and implementation of advocacy for families.
  • Ensure preparation with the input from team, family, existing assessments and evaluations, identification of priorities and plan of care within 60 days, assist family in obtaining additional evaluations as necessary.
  • Maintain CANS certification on a yearly basis.

· Provide services to families with children who have special needs with accessing needed services through collaboration with other agencies and service providers while providing advocacy and support for families when needed.

  • Determine the range of services needed through interviews, home visits, collateral contacts, etc.
  • Prepare case related documents including individual services plans, reviews, forms, correspondence, HSRS/PPS reporting, and case record dictation.
  • Coordinates services with other agencies/disciplines/community members such as schools, DVR, medical personnel, therapists, nursing homes, Social Security Administration, family members, guardians, etc.
  • Authorize expenditures of funds (with supervisor approval) within identified service plan and monitor expenditures of funds and monitor expenditures for effectiveness and efficiency and ensure against over-expenditure of funds.
  • Other duties as assigned.

Job Type: Full-time

Pay: $21.47 - $23.47 per hour
