Job Description Summary

The Certified Nursing Assistant provides skilled and non-skilled care to patients in their homes under the supervision of the RN according to the care plan and physician's orders, when applicable. These services will meet the agency's standards within the scope of licensure requirements and following established standards and practices.

Pay: $18-$20

This position: PRN 5:30am-10:00am


Essential Job Functions/Responsibilities

1. Requires full range of body motion including handling, lifting, and transfer of patients.

2. Manual and finger dexterity and eye-hand coordination.

3. Occasionally lifts and carries items weighing up to 50 pounds.

4. Requires corrected vision and hearing to normal range.

5. Requires working under stressful conditions and working irregular hours.

6. Knowledge of infection control is required.

7. Requires a current driver's license and the ability to drive to and access a patient's home without an assistive device.

8. Current vehicle insurance and license is required.

9. Background check is required.

Position Qualifications

1. Currently licensed as a Certified Nursing Assistant within the guidelines of the State in which you are currently practicing.

2. One year experience as a CNA with a home health agency or in a long-term care facility preferred.

3. CPR certification required.

4. Must have strong verbal and written communication skills.

5, Experience working in Home Health preferred.

6. Must be a critical thinker, with a skill in identifying problems and recommending solutions.

7. Excellent attention to detail.

8. Excellent in preparing and maintaining records, documentation, and responding to correspondence.

9. Skill in establishing and maintaining effective relationships with patients; clinical staff, and the public.

Job Duties

1. Observes, records, and reports to the RN Case Manager the general physical and mental condition of patient.

2. Notifies RN Case Manager if the patient's condition worsens or changes.

3. Assists in the development of the plan of treatment.

4. Performs procedures and treatments according to the needs of the patient and as indicated on the written plan of treatment.

5. Provides reminders and assistance for prescribed medication and treatments according to law, standard practice, and as permitted within the scope of licensure.

6. Document CNA tasks in the patient's chart for each visit via KanTime mobile.

7. Safely and appropriately performs activities of daily living as written in the plan of care which may include bed bath, shower bath, hair shampooing, nail and skin care, oral hygiene and toileting/elimination.

8. Teaches self-care techniques to the patient and /or family.

9. Complete 12 hours of in-service training annually.

10. Assists the RN/LPN in performing specialized procedures, as requested.

11. Follows all agency policies and procedures.

12. Performs other related activities as necessary.

13. Maintains patient confidentiality.

14. Turns in notes and timecards according to policy.

15. Accepts assignments based upon specific patient needs and family capabilities.

16. Shows interest in learning new skills.

17. Is knowledgeable about community resources.

18. Is able to work with Managing Nurse to initiate appropriate preventative and rehabilitative procedures.

19. Attends required meetings.

20. Has good rapport with peers.

21. Accepts direction from RN Case Manager, clinical Manager, and DON.

22. Uses Universal Body Fluid Precautions and follows OSHA guidelines at all times.

23. Is supportive of administrative staff decisions of the agency.

24. Consistently wears nametag and is dressed per agency policy when seeing clients.

25. Performs non-medical duties as needed. These can include safely assisting patients entering or leaving bed or bath, assisting patients walking, normal range of motion and positioning, tidying patients' rooms, make beds, change linens, light housework, laundry, provide patient hygiene, food prep and feeding assistance and any others as indicated in the care plan.

NOTE: Certified Nursing Assistants will be supervised by the Clinical Manager and RN Case Manager. CNA will be paid on a per-hour basis for patient care, for staff meetings, and in-services. Salus of Utah cannot guarantee any number of hours in any given week or the same number of hours each week. Mileage will be paid according to policy.

Typical Working Conditions

Majority of work is performed in providing patient care services in the patient's home. Exposure to communicable disease is common.
