POSITION SUMMARY: To build and maintain community relationships specific to clientele and potential referrals.

EDUCATION/EXPERIENCE/CERTIFICATION:

• High School Education
• Peer Support Certified in KY
• Preferred: Associates (+) Degree

Role

• Building community relationships: Attend monthly meetings with Referral Sources. (DCBS, Probation and Parole, Health Department, Incarceration Centers, Local Hospital Social Worker, KVC, Homeless Shelters, Women’s Shelter, Pretrial Services, Drug Court, Goodwill Communities, Education Program Advisors)
• Billing in EMR system, Peer Support individuals to discuss communication updated from their referral sources.
• Coordinate with Residential centers the res to op handoff.
• Participate in community meetings advocating for OP Services.
• Advocating for clients with referral sources such as attending court dates, and speaking with probation/parole about client progress/concerns.
• Work in partnership with the Treatment team to confirm client progress/concerns.
• Ensure all referral sources are accurately input into the EMR system.
• Participate in Staffing at each OP Center once a month.
• Ensure compliance letters are completed for referral sources.
• Ensure tracking sheets are up to date.
• Contacting liaison/office centers to set up clients for intake.
• Visit Residential Centers to present Outpatient services and good time incentives from Department of Correction opportunities.
• Work directly under the OP Regional Director.
• Document all contacts/meetings in the format that is advised by OP Regional Director.
• Provide peer support services as a fill-in when needed.
