POSITION SUMMARY

The Community Health Evaluation and Data Coordinator supports team members in building structures, best practices, and hands-on training with staff to support overall data input, management, and reporting. The coordinator extracts data and conducts data analysis for grant reports, also creating data visuals and presentations to engage staff in data reflections. They also develop data collection tools (surveys, interviews, focus groups, etc.) with staff and lead staff in evaluation processes. In addition, the position provides training for data tracking and management, and creates data monitoring tools. This position is a hybrid role, functioning 60% remotely and 40% on-site/community-based.

ESSENTIAL DUTIES AND RESPONSIBILITIES (Included but not limited to the following);

· Works closely with population & community health team members to organize and input data from health services and programs.

· Works hand in hand with population & community health team members and community stakeholders to co-create evaluation processes and build capacity and organizational support; supports teams in developing evaluation questions to capture complex initiatives and evaluate social impacts.

· In collaboration with the Director of Population & Community Health, drives evaluation processes including the methodology and logical frameworks for developing goals, indicators, and outcomes.

· Supports data and compliance tracking for grant reports and team goals.

· Supports with administration, technical assistance, monitoring and maintenance of the data which includes, but is not limited to: database training, developing database workflows/manual, cleaning for duplications, identifying missing data, maintaining historical data, data migration, developing data dashboards/reports, adjusting user access and data privacy settings, etc. for the community health team and the organization at large under the guidance of the Director of Population & Community Health.

· Coordinate with FQHC leadership to develop/maintain a fully integrated FQHC outcome measures report with oversight responsibility for data collection and presentation of FQHC Core measures, including but not limited to program specific measures.

· Act as a data quality liaison for EMR and HIT workgroups and collaboratives as invited or assigned.

· Develops reports, tracking tools, and dashboards for data management.

· Cleans and analyzes data; develops charts, graphs, and other data visuals to present to the team/partners; calculates community health statistics and changes over time to help determine impact.

· Researches community health population data (census, city, state, national, etc.); assists in data collection for needs assessments; keeps abreast of new trends in data, inventory of external data resources, validity of existing repositories of data, etc.

· Supports Evaluation and Data team in organization-wide projects towards developing organizational capacity for data collection and evaluation such as: database improvements, logic models, and Continuous Quality Improvement (CQI).

· Analyze outcomes data and make recommendations for improvements and efficiency based on data collection.

· Support organizational data requests, provide support (statistical, computer hardware and software) for statistical analysis and graphics applications
Assist with data abstractions/analysis and preparation of presentations and or grant applications and reports

· Assist in data collection and submission of annual HRSA UDS report

· Attends partner, community and staff team meetings to stay current on initiatives and activities in the community.

EDUCATION AND EXPERIENCE

· Bachelor’s Degree in public health, community health, health sciences, epidemiology, health administration or equivalent experience and education (2-4 years).

· Experience using EMR systems for data extraction purposes.

· Experience with implementing and maintaining electronic recordkeeping and communications systems in the healthcare environment.

· Must have experience using EMR/EHR systems (Athena is a plus) and data visualization software (Tableau, Power BI).

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
