MAIN FUNCTION:

The Community Health Worker collaborates with the care team to create and improve the integrated Case Management Program (iCMP) system of care. The Community Health Worker works with the care team and family to ensure patients are receiving the services they need to achieve an optimum quality of life and serves as an advocate for the family, teaching them how to navigate the health and mental health care systems as well as community services including social and educational systems to ensure patients are functioning to their full potential by receiving services along the continuum of care. The Community Health Worker partners with families to help them organize their activities to follow through with essential steps in their health and carries a caseload of patients in conjunction with an RN/SW Care Manager. The Community Health Worker meets the patient and family’s needs efficiently and expeditiously by continuously improving the patient and family experience and helping to ensure the standards of high quality patient care.

This position reports to the Director, Clinical Operations and operates within established organizational and departmental policies and procedures.

MINIMUM REQUIREMENTS:

• High school diploma or equivalent preferred; relevant experience in the community or Associates Degree required; Bachelor’s Degree preferred
• 1-2 years of prior experience working with patients and families dealing with complex medical and psychiatric/behavioral problems; the ability to help patients and families/caregivers understand and access the resources required to support care, ideally with experience in private/public educational systems, community mental and behavioral health centering on holistic patient and family care required
• Bilingual preferred
• Experience as a community health worker, peer specialist, peer advocate, or a similar role preferred; firsthand or secondhand experience dealing with behavioral health issues, navigating the behavioral health care system, and/or navigating the special education system in Massachusetts preferred
• Strong understanding of psychiatric and family system and ability to use this understanding to formulate succinct case summaries required
• Good organizational and time management skills and the ability to set up systems to help others with these skills required
• Strong computer skills and use of Microsoft Word/Excel/Outlook required; experience with electronic medical records preferred
• Strong interpersonal skills enabling effective team collaboration required
