Community Health Worker III

Sinai Urban Health Institute (SUHI) is hiring full-time, culturally competent Community Health Workers (CHWs) to work at clinic and emergency room locations on Chicago’s South Side. This role is part of the South Side Healthy Community Organization (SSHCO), which is comprised of 13 healthcare organizations including safety net hospitals, health systems and Federally Qualified Health Centers. The model will better connect existing health organizations, increase access to care, address some of the most challenging health issues, and ultimately, build health equity and ensure stronger, healthier communities across the South Side of Chicago.


Under the direction of the CHW supervisor, the CHW is responsible for providing and coordinating health education and disease management education, assisting individuals with navigating the health and social care systems, and providing case management services (under supervision). The CHW works within an interdisciplinary team and serves as a bridge between the patient and the medical system by building trusting relationships with community members served by the health center or program(s). The position may coordinate the work of CHW Is and IIs, assist in their training and mentoring, assist the CHW supervisor with research duties, oversee complex case management situations, coordinate outreach activities, and facilitate community relationships. Home visits may be required for some interventions. The position is grant funded and salary range is $20-23/hour, based on experience. The role is non-remote; onsite attendance during the day shift is required Monday-Friday.


SUHI is seeking an enthusiastic, passionate CHW candidate who meets job requirements outlined below.


Position Requirements:

  • Must have high school diploma or GED; some college or a degree is preferred but not required
  • At least 3 years of relevant experience in the areas of community health, health education, management, administration and training (with at least 2 years of direct community health experience)
  • Demonstrated leadership and mentorship experience for at least one year
  • Candidates residing in a South Side community are preferred
  • Passion for and dedication to working with community members
  • Punctual, reliable and willing to learn
  • Proficiency in Microsoft Word, Excel, and PowerPoint preferred
