Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary: Potential Work From Home (Hybrid Schedule) Mon-Fri 35hrs.

Responsible for coordination between DMHAS services and the private sector to facilitate the transition and integration of DMHAS consumers with brain injury into community-based supports and services.

Essential Duties & Responsibilities:

  • Accepts referrals from DMHAS ABI Services Clinical Supervisor. Completes a comprehensive biopsychosocial assessment to identify appropriate supports and services to address the client’s needs.
  • Provides community integration services for DMHAS ABI consumers residing in the catchment area covered by DMHAS Region 4. Serves as an active team member. Assists in all aspects including waiver application, coordination with consumer, family and/or conservator, etc. Makes site and home visits as needed.
  • Acts as liaison to all public and private providers and advocacy groups that are engaged in serving DMHAS ABI consumers. Facilitates communication among team members. Provides necessary information for discharge planning including funding and development of community-based individual service plan, application for benefits and the ABI Medicaid Waiver when appropriate.
  • Serves as liaison between DMHAS ABI Services and inpatient facilities. Possesses thorough knowledge of DMHAS, CVH, InterCommunity and related programs, services and facilities. Demonstrates the ability to facilitate open and productive communication among all parties involved in discharge planning and community inclusion.
  • Facilitates transition into the community for consumers with ABI. Gathers input from consumers and families regarding consumer choice and program development. Works cooperatively with inpatient facility. Attends team meetings. Gathers necessary information including records required for adequate service provision and funding. Facilitates application for public benefits as appropriate. Completes community assessment and produces written consultation and reports as required.
  • Gathers programmatic and statistical data as required. Produces reports as required. Maintains current documentation in accordance with United States and DMHAS ABI Services requirements.
  • Demonstrates commitment to professional growth and development as evidenced by attendance at educational programs as requested and to secure and maintain a CT LCSW, as appropriate. May provide in-service training to others.
  • Complies with all State and Federal regulations and related standards, procedures and policies, as well as DMHAS and InterCommunity Policies and Procedures.
  • ABI training provided.
  • Performs other duties as assigned.
Requirements:

Education &/Or Experience:

CT LCSW, LPC or LMFT from an accredited school required .

Qualifications:

Demonstrates a basic knowledge of brain injury, mental health and substance abuse, and the services, programs and entitlements available in Connecticut. Must have current Connecticut Driver’s License and ability to drive own vehicle throughout catchment area. Bilingual (English/Spanish) and/or ASL a plus. Experience in working with various systems.
