Job Overview:*
The Community Outreach Specialist assists Wellpoint Care Network staff and Clients in identifying community partners that may help strengthen client’s needs under the 5 pillars of stability: health, housing employment, education, and caring connections. The Specialists provide consultation in conducting and documenting assessments of family needs in the five pillar areas, for each family served by Wellpoint.

Qualifications:*

  • Bachelor’s degree in social work or related degree required
  • 2 years’ work experience in child welfare or social service setting with a preference for work with community agencies.
  • Experience in relationship-building with both internal and external partners.
  • Understanding of and experience in working with diverse communities and families.
  • Excellent problem-solving skills
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Must be honest, dependable and able to meet deadlines; Self-motivated and able to work independently.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.

Duties:*
Community Outreach and Relationship Building- Maintains current community partnerships based on the ongoing needs of families served by Wellpoint as well as proactively seeks out new community partnerships based on the identified needs of children and families in the surrounding neighborhoods.
Family Assessment and Delivery of Direct Services - Performs ongoing needs assessment with individual program areas as requested based on the five pillars of family stability: education, employment, health, housing, caring connections.
Program Growth and Development - Takes an active role in Agency and program development activities to help improve the quality of services offered by Wellpoint and enhance Agency growth and community partnerships.
Record Maintenance - Develops and maintains internal resource directory of community partners and resources.
