Valley Professionals Community Health Center is seeking a Community Resource Navigator for our Clinton and Cayuga locations.

The Community Resource Navigator will be responsible for assessing patient needs, identifying, navigating, and accessing community services to meet the needs. This position will assist eligible patients in applying for publicly sponsored health care insurance plans; conduct outreach in the community; and provide information on available resources.
Job duties:
- Supports the Mission of the Valley Professionals Community Health Center;
- Initiates follow-up with new health center patient to assess patient needs and review available services;
- Conducts “in reach” activities with health center patients that have missed or no showed appointments to identify barriers, assist with rescheduling and mitigating obstacles to future health services;
- Coordinates transportation for patients with need;
- Assists families, groups and communities in accessing health related services;
- Provides assistance in obtaining prescriptions through pharmaceutical company programs or 340B;
- Assesses established patients for needs and refers patients to community resources and other agencies to meet identified needs, providing necessary follow-up to ensure needs are met;
- Attends community-based events and health fairs to educate on available services through Valley Professionals;
- Engage patients, families and caregivers in healthcare and support services;
- Assist patients in setting self-management goals in coordination with RN Care Managers;
- Delivers screenings and education to target populations;
- Completes and maintains Community Health Worker certification;
- Complies with all federal and state laws as well as all VPCHC policies, procedures, rules and regulations;
- Complies with all HIPAA rules, including but not limited to, the Privacy Rule, the Security Rule and the Breach Notification and Enforcement Rules; and
- All other duties as assigned.
Requirements:
- High school diploma or equivalence required.
- BA or BS in community health, health care administration or other health care field preferred.
- Experience in a public or private health or social service agency preferred.
- Understanding of individual’s socioeconomic status, culture, and lifestyle with respect for individual differences.
- Maintain a valid driver’s license and ability to travel between health center sites and throughout the health center’s service area.
- Ability to successfully complete all required and applicable federal and/or state consumer assistance training and obtain license as Indiana Navigator and Certified Application Counselor.
