Compensation:

VOANS uses a pay scale based on years of experience $30.78-$34.90

About the Job: Take advantage of the opportunity to obtain highly specialized training, continuous education, and utilize technology-driven assessments to improve residents’ incontinence.

This position combines best practices along with new emerging technology to formulate holistic care plans targeting incontinence in skilled nursing facility residents. You will be trained in innovative technology that allows for more accurate, objective assessment data. In collaboration with the care team, you will then use this data, along with resident preference, to create individualized care planning strategies.

You will be provided with, and be responsible for sharing with your team, the most up-to-date, innovative, expert knowledge on incontinence in the late-life population.

The CIS will use their unique knowledge of late life care needs to support a balanced approach to care using specific care strategies to avoid unnecessary decline while recognizing inevitable age-related decline.

Essentials:

  • Contribute to Work of Aging care planning with a balanced approach to biological, psychological, and social late life needs of residents.
  • Conduct assessments/observations to identify root cause of incontinence through utilization of technological resources (eg. Tena Identify, high functioning bladder scanner, Actigraphy, etc.)
  • Execute on-site Strategies Targeting Resident Elimination Assessment and Management (STREAM) initiatives with Empira direction and guidance.
  • Increase awareness of the STREAM grant initiatives within the community (residents and staff).
    Partner and collaborate with staff development to provide education.

Required Qualifications:

  • Extensive experience in a health care setting
  • Must be a Registered RN in the state of MN
  • Must be comfortable acting as an agent of change within the facility
  • Demonstrates an ability to utilize computer systems and applications.
  • Must possess strong belief in the importance of quality improvement and act as a role model for desired quality competencies. Commitment to following concepts of QAA and the QAPI process.
  • Effective oral and written communication skills and teaching and coaching skills.
  • Ability to be self-directed
  • Understanding of current Antibiotic Stewardship, Infection Prevention and Infection Control and OSHA guidelines.
  • Knowledge of state and federal requirements for long term care.

Preferred Qualifications:

  • A bachelor’s degree in a health care related field is preferred
