Job Description


PURPOSE OF POSITION:
To organize and manage population health and access data as well as dissemination thereof to senior and departmental leadership. To collect, organize, and facilitate the collection of PCMH related components. To ensure through the design of standard work, reporting tools, training materials, cross functional meetings and others managerial processes that PCMH requirements are well known and implemented in a manner sufficient to ensure accreditation.

AREAS OF RESPONSIBILITIES
Responsible for organization and management of all centrally developed population health and access data as well as dissemination thereof to senior and departmental leadership. Responsible for support to leadership and frontline staff as needed in use of population health tools designed to guide patient care, such as the Population Health Dashboard + Explorer, the CDMT, and High Risk Registries. Responsible for management of Epic reports corresponding to key ambulatory care access metrics. Responsible for facilitating regular understanding of, access to, and regular use of the Care Gap tool by administrative and clinical leaders as well as frontline staff. Responsible for facilitating understanding and completion of required PCMH components. Other reports and functions as needed.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

1.Under direction and with wide latitude for the exercise of independent initiative and judgment, develops a deep understanding of and capacity to use centrally developed population health management tools in order to support departments and frontline staff in their regular use. Tools include:
  • Population Health Dashboard
  • Population Health Explorer
  • Chronic Disease Management Tool (CDMT)

2.Under direction and with wide latitude for the exercise of independent initiative and judgment, facilitates regular use of the Care Gap Tool by department leaders and frontline clinical teams to close critical gaps in care, translating to increased revenue associated with Managed Care value-based incentive payments.

3.Oversees Patient Centered Medical Home (PCMH) day-to-day operations, and monitors and pulls relevant data needed to sustain PCMH.
4.Supports departments in complying with PCMH regulations.
5.Responsible for dissemination of/access to registry-driven, patient-level data to/by clinical care teams to support outreach.
6.Serves as Gouverneur’s primary liaison with Gotham PCMH, facilitating the sharing of questions and information on behalf of clinical and administrative staff.
7.Maintains metrics, generates reports, and tracks data and progress of the facility’s population health and access data.
8.All data, logs, worksheets, and other documents are maintained on a daily, weekly, monthly or quarterly basis throughout the year.
9.Facilitates and participates in projects and tasks relevant to operational workflows that align with the guiding principles of Gotham Health and Ambulatory Care.
10.Assists with the design of policies and standard work that serve to ensure the completion of regulatory guidelines.
11.Designs training curriculum and materials; trains and educates staff as needed in PCMH related job duties: Smart phrases, care plans, MyChart video visits, etc.
12.Schedules and facilities meetings as needed.
13.Tasks, projects, and other assignments are completed fully and accurately as directed by assigned due dates.
14.Maintains the provider/AOD on-call schedule for all Ambulatory Care practices.
15.Maintains the Care Team Workbook for primary care practices.
16.Any and other duties as assigned by the Chief Transformation Officer or supervisor.

Minimum Qualifications


1. A Master’s degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling; and
2. Two (2) years of full-time experience in medically-oriented health care and medical support systems environment, one (1) year of which included experience in an administrative or supervisory capacity in such occupations as physical, occupational, inhalation, speech and audiology therapy or as Medical/Laboratory Technologist, Microbiologist (various specialties), Chemist (biochemist), Radiation and X-Ray Technician; or two (2) years of responsible level experience in business management systems or general administration including one (1) year in a supervisory or administrative capacity; or
3. A Baccalaureate degree from an accredited college or university in disciplines listed in “1” above; and
4. Three (3) years of full-time experience, as described in “2” above, including two (2) years of experience in an administrative or supervisory capacity in occupations, as listed in “2” above; or
5. A satisfactory equivalent combination of education, training and/or experience.

Note: Certification or licensure may be required.

EDUCATIONAL LEVEL:
1.A Baccalaureate Degree from an accredited college or university in Business Administration, Social Sciences, Management, Health Care Management or related discipline, or
2.A satisfactory equivalent combination of education, training, and experience.

KNOWLEDGEABLE IN:
1.Competence in computerized systems related to electronic medical records, scheduling, and analytic systems.
2.Strong organizational and time management skills.
3.Ability to work well with others and communicate effectively.
4.Demonstrates effective interpersonal, verbal, and written communication skills.
5.Strong analytical skills and detail-oriented.

YEARS OF EXPERIENCE:
At least 3 years of experience in a clinical setting, or equivalent.
