Coordinator Care 7998 Job at RIDGEVIEW MEDICAL CENTER
Purpose of Job
This position is responsible for completing in-home health risk assessments for Ridgeview’s senior population.
Job Functions
- Performs face-to-face initial, annual and change of condition assessments; more frequent contact based on patient complexity and risk
- Supports patients in establishing a Primary Care Provided (PCP) and collaborates with PCP and care team to create patient-centered Care Plans.
- Be a the first point of contact for patients with health/social concerns (other than emergencies).
- Connects patients with community-based resources.
- Attends training with payer clinical liaison team and MN Department of Human Services (includes becoming a Certified Assessor through DHS).
- Performs other duties as assigned.
Minimum Education/Work Experience
- Bachelor's degree in healthcare-related field
- At least one-year experience as a nurse, care coordinator, case manager, or social worker
- Strong customer service experience
Preferred Qualifications
- Licensed Social Worker or Registered Nurse
- Certified MN Choices Assessor
Knowledge/Skills/Abilities
- Ability to meet and maintain the necessary background checks as aligned with position functions
- Ability to serve patients in a friendly and efficient manner.
- Knowledge of basic computer functions and Microsoft Office applications and database management.
- Capable of making independent decisions with the use of good critical thinking and problem-solving skills.
- Ability to respond appropriately to unpredictable situations.
- Knowledge of medical terminology, diagnostic procedures, and tests.
- Knowledge of HIPAA requirements.
- Ability to communicate in the English language for effective written and verbal correspondence to complete job functions as mentioned above.
