Job Title: Dementia Education Community Specialist / Intake Coordinator

FLSA Status: Non-Exempt

Salary Range: See Salary Scale

Job Summary: The position reports to the Director of Community Outreach. Primary responsibility is to provide consistent dementia education to Home Care Agency (HCA), participant caregivers and the general community. This dual responsibility will provide for the on-going growth of referral sources and recruitment and enrollment of PACE participants throughout the community to increase enrollment of PACE participants.

Specifications

Education: Bachelor’s degree or equivalent experience, preferably in a health care field.

Experience: Minimum of 1 year experience working with the frail elderly population. Minimum of three years of professional level experience in community health services and/or dementia care/education. Experience in independently developing, maintaining, coordinating, and carrying out promotional events, internal information programs and public information campaigns required.

Licensure, Registry or Certification Required: None

Special Training: Must be able to effectively work in a team environment. Must possess excellent communication, interpersonal and conflict resolution skills. Knowledgeable of Medicare and Medicaid preferred. Trained in the care of the frail elderly diagnosed with dementia.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Neonate/Infant Early Childhood Adolescent Adult Geriatric All Age Groups

Key Responsibilities: (*denotes an age-related skill or task)

  • Provides monthly dementia education to all staff, to include the POSP.
  • Assists with dementia education and training for the staff in the expanded space.
  • Evaluates potential enrollees to determine participant needs and eligibility for enrollment.
  • Participates in the IDT and other related meetings and activities as it relates to efficiently communicating and working with the IDT when evaluating potential enrollees.
  • Assists with the development, implementation, and evaluation of the overall marketing strategy for the recruitment and retention of PACE participants.
  • Assists with the operation of new participant orientation.
  • Provides backup duties and responsibilities for current center managers and facilitates care plan meetings as needed.
  • Builds and maintains current community relationships throughout the service area through dementia education and community engagement.
  • Creates dementia education opportunities with senior groups, churches, and senior centers.
  • Serves as Chair the End-of-Life Committee.
  • Serves as Chair the Diversity and Equity Committee.
  • Serves as a liaison between POSP and new contracted providers. Educates potential contract providers on the PACE Model and provides annual and ongoing training as requested.
  • Serve as liaison for the Life Enrichment Center.
  • Supports Pace of Southern Piedmont's mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
  • Other duties as assigned.
