BCP is a nonprofit pediatric private practice that opened its doors in November 2020 as the first of its kind in Massachusetts. Its mission is to bring equity to pediatric healthcare by offering underserved patients direct access to their primary care provider through traditional medical office visits, telemedicine, and mobile medical services while providing comprehensive, prevention-oriented healthcare, care navigation services, and integrated behavioral health services.

About the Director of Care Navigation and Wellness

BCP is currently seeking a compassionate leader to join our team as the Director of Care Navigation and Wellness as we continue to break down silos and decrease stigma by sharing the burden of coordination with families and providing truly integrated care, in which medical providers, mental health clinicians, and care navigators and family partners are not just co-located, but are actually working together. The Director of Care Navigation and Wellness will have primary responsibility for leading the team and ensuring that all patients and families receive the highest level of support through care navigation, wellness programming and our special events. The Director of Care Navigation and Wellness will report directly to the Founder.

Integrated Care Navigation and Wellness services are a critical component of BCP’s successful model in centering patients and their needs in all that we do as we work to improve the way healthcare is delivered to all children.

Key Responsibilities

Leadership

  • Provide leadership and guidance over all aspects of the Care Navigation and Wellness program; ensuring that it continues to exemplify the vision of BCP
  • Continually assess the impact of the various programs and services; modify as necessary
  • Work closely with all providers to understand the evolving needs of our patient population
  • Recruit, develop and supervise the Care Navigators
  • Recruit, develop and supervise interns and volunteers to support programming needs

Casework

  • Provide direct individual care navigation services to a small caseload of patients
  • Provide support, guidance and direction to care navigators on individual cases as needed
  • Work closely with BCP’s family partner around MassHealth case management program
  • Participate in Daily Rounds and bi-monthly High Risk Rounds

Community Engagement

  • Partner with community organizations to support resources for patients including USES, Hope and Comfort, YMCA, and others
  • Develop programming for community organizations in conjunction with providers and Director of Mental Health Services to support BCP’s Hyperlocal Community Initiative
  • Develop new partnerships and work with existing community partners to support the needs of families

Programming

  • Oversee the daily after-school activities, primarily over Zoom - dance, karate/fitness, reading, Girls on the Run (operates Spring and Fall) and tutoring
  • Develop and oversee summer programming; utilizing summer interns for day to day leadership and implementation
  • Oversee weekly cooking class which includes development of weekly recipes, coordination of food delivery for families prior to class, facilitating class over Zoom with assistance of other care navigators/volunteers
  • Lead biweekly meeting with coordinators of tutoring and reading programs to ensure programs are running smoothly and there are no patient concerns
  • Recruit patients for programs in conjunction with providers and BCP staff
  • Oversee various special events throughout the year in conjunction with BCP staff: camp night, back to school event, Thanksgiving turkey distribution and Holiday store including outreach and engagement with families

Grant Reporting

  • Oversee Children’s Hospital Fitness in the City Grant including data collection that occurs at pediatric visits
  • Provide oversight of new grants such as; Food Distribution within the No Kid Hungry Grant
  • Assist with grant writing as needed

Qualifications and Skills

  • 5+ years of experience working with children and families
  • Openness to and respect for others’ cultures, identities, and lived experiences
  • Highly organized with ability to effectively prioritize diverse responsibilities/tasks in a fast-paced environment. Detail-oriented with the ability to complete work accurately and efficiently
  • Superb interpersonal and communication skills
  • Unwavering commitment to quality programs and data-driven program evaluation
  • Excellent teamwork skills
  • Bachelor’s degree with MPH or MSW preferred or equivalent experience required
  • Interest/experience in leading data-driven program evaluations
  • Proficiency in Spanish preferred, but not required
