Developmental Pathways has an opportunity for a Family Support Services Coordinator (FSSC) in our Case Management department who will verify eligibility and develop, monitor, and assure implementation of the Family Support Plan (FSP). The Family Support Services Coordinator will assure services and supports identified in the annual support plan are coordinated among various providers for continuity of services and funding for individuals served. A Family Support Services Coordinator is an empathetic, collaborative, and action-oriented employee with a desire to make an impact in the community and within our teams.
This is a hybrid position, working remotely and coming into our headquarters office in Aurora, CO and/or off-site locations, as needed. This is a full-time position at 40-hours/week.
Starting Salary and Benefits
$22.95-$24.59 hourly or $24.00-$25.64 hourly for Bilingual Case Manager roles

Requirements
  • An Associate’s Degree or equivalent experience in human services related field
    • OR five (5) years’ experience in the field of developmental disabilities, or combination of education and experience appropriate to the requirements of the position
  • Ability to meet and maintain our driving requirements; no suspensions and less than three (3) tickets in three years
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance
Preferred
  • A Bachelor's Degree
  • Knowledge of rules and regulations for Health Care Policy and Financing and Colorado Department of Public Health and Environment
  • Two (2) years’ experience in service coordination/case management at Developmental Pathways or similar agency
  • Familiar with Microsoft Office (Excel, Word, Outlook, etc.) and be able to use these and other required programs at an intermediate level
Additional Requirements for Bilingual
  • Able to speak, write, and interpret fluently in English and another language based on the business needs
  • Ability to complete all typical case management responsibilities in fluent language/s
  • Support with written translation of internal documents for use by stakeholders, as appropriate
Additional Responsibilities
  • Facilitate and coordinate Interdisciplinary Teams (IDTs), document and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary team members
  • Coordinate support services for adults and/or children living in a variety of settings
  • Communicate with a broad team of providers which may include professionals, direct care employees, families, and natural supports chosen by the individual/family being served
  • Assist with department initiatives and outreach efforts
  • Provide quality family support services to individuals applying for, and/or receiving services, and their families
  • Research, request, and obtain potential services and/or providers to meet individual needs/preferences as determined in the FSP
  • Complete Allocation Requests and Check Requests for individual/family’s needs; reviews and assures expenditures are within budgetary guidelines
  • Serve as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases
  • Complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools
  • Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies
  • Complete accurate case documentation in appropriate tracking and databases; prioritizes workload and meets all required timelines
  • Maintain knowledge of state/ federal regulations, policies and procedures regarding long-term services and supports, Medicaid waiver and state funded programs and other community resources
