Country Hills Post Acute is a leading Skilled Nursing and Rehab Facility located in El Cajon, San Diego. We are one of the largest Skilled Nursing Facilities in the San Diego Area. The essential job functions of a Speech Therapist include the following:

- Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral.

- Develop effective treatment plan and obtain approval for services from referring physician.

- Communicate with supervisor and other health team members regarding patient progress, problem and plans.

- Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.

- Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech and Language Pathologist in the Board State Practice Act and governmental and third party payer requirements.

- Instruct patient’s families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.

- Participate in discharge planning.

- Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Qualifications:

- Bachelor’s degree in Speech-Language Pathologist Therapy, prefer graduates of Masters or Doctorate Program in Speech-Language Therapy.

- Must have exceptional communication and customer service skills and be empathetic.

- Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care.

We are looking for candidates who can demonstrate the ability in maintaining confidentiality of patient and facility information, prioritize responsibilities within the allotted time, independently make decisions when warranted and remain calm in emergency situations, communicate effectively and deal tactfully with residents, family members, and the public, and demonstrate honesty and integrity at all times.
