Provides individualized self-management education and training to individuals and groups according to the scope of practice. Group Leaders maintain a non-technical role in providing instruction to program participants. This position requires full understanding and active participation in fulfilling the mission of the organization. It is expected that the employee will demonstrate behavior consistent with the core values.

Qualifications
Minimum Qualifications:
- Experience in community health education, clinic or acute care or, promotora or community health worker , health and wellness, life style coach or support group facilitator.
- Prior medical field experience and knowledge of medical terminology
- Able to work in a culturally and socio-economic diverse environment with professionalism and confidentiality.
- Bilingual in English and Spanish
- Bilingual in English and Hmong
- Must possess and maintain leader certification in one or all of the following: CDSMP, DSMP, Tomando Control de Su Salud, Manejo Personal de la Diabetes, Walk With Ease, Matter of Balance and DPP or maintain certification within 1 year of hire.
- Must possess and maintain a valid California Drivers license and proof of auto insurance.
- Must possess and maintain a Basic Life Support (BLS) certificate.
Pay Range
$18.83 - $25.89 /hour
