The Health Advocate is a trusted member of the community they serve. They have a good understanding of the health resources available in their communities to refer patients to the best resources available to them. This position reduces health disparities, improve patient engagement with their health, enables patients to get the care they need and improving health outcomes. You will be responsible for educating patients about their rights, insurance coverage, and payment options, as well as addressing any queries or complaints. Your ultimate duty is to ensure patients receive the best care available to them.
- Make face-to-face and telephonic connections and build rapport with patients.
- Serves as a liaison between patient and care providers.
- Participate in and consult to inter-disciplinary care teams to support complex members who have resources needs or logistical barriers.
- Documenting and resolving any complaints reported by patient or caregiver.
- Explaining test results, diagnoses, and available treatments to the patient.
- Communicating information to patients in an understandable manner so that they can make an informed decision about their healthcare.
- Establishes collaborative relationships with staff from various agencies to promote collaboration.
- Resolve access to care barriers by closing care gaps.
- Participate in on-site events, clinics, and outreach initiatives as assigned.
- Ensures confidentiality of patient information by creating and maintaining a secure and trusting environment.
- Identify patients who are overdue for visits, labs, and referrals, contact patients and arrange for follow-up services.
- Perform other duties as assigned.
Experience with EHR and/or Athena a bonus.
Job Type: Contract
Salary: Up to $20.00 per hour
Schedule:
- 4 hour shift
- 8 hour shift
- Weekends only
Ability to commute/relocate:
- Atlanta, GA 30344: Reliably commute or planning to relocate before starting work (Required)

- Driver's License (Preferred)
