Primary Accountability

The Health Educator is responsible for collaborating with patients, family members, health-care team members, health-care team members from other organizations, and other stakeholders to reduce or eliminate barriers to care.


Description of Primary Responsibilities

1) Builds and maintains relationships with patients and other stakeholders for the purposes of improving quality of care.
a) Such activity will include, but not be limited to, patients and their family members who have:
i) Chronic illness(es) that typically require close adherence to an ongoing treatment plan to be effective;
ii) Histories of avoidable emergency department visits;
iii) Prenatal needs; and
iv) Other concerns or conditions that have socially mediated causes or influences that act as barriers to improving health.
b) Health Educators interface with patients and other stakeholders through a variety of mechanisms, including, but not limited to:
i) Individual, face-to-face contacts through both appointment and warm hand-off contacts;
ii) Leading scheduled classes/groups within FHCN Health Centers and in the surrounding communities;
iii) Telephone and other electronically mediated contacts; and
iv) Contact outside of FHCN Health Centers at community functions such as health fairs.
2) During contacts with patient and family members, Health Educators reduce barriers to care in a number ways including, but not limited to:
a) Collaborating with patients and their family members in understanding, setting and monitoring self-management plans;
b) Assisting patients with finding resources, services and support mechanisms for their care plans and self-care management needs;
c) Provides the patient with educational materials and resources appropriate to the health literacy needs, including assisting patients in understanding how to successfully access care both within and outside of FHCN; and
d) Assisting patients with assessments, orientations, educational plans, referral processes, and forms completion relevant to programs that serve special populations including but not limited to:
i) Comprehensive Perinatal Services Program, including high risk perinatal patients;
ii) Title X and Family PACT services; and
iii) Cancer Detection/Every Woman Counts program.
3) During contacts with health-care team members at FHCN and from other organizations, Health Educators reduce barriers to care in a number of ways including but not limited to:
a) Preparing, printing and distributing the information necessary for care teams to engaging in Morning Huddles;
b) Maintaining and servicing panels of patients with special service needs, as determined by health center or organizational need;
c) Maintaining regular communication with care team providers on patient care plan goals and progress;
d) Facilitating regular communication between patient and other health-care team members both inside and outside FHCN; and
e) Providing staff training and education sessions necessary to implement health education services.
4) Participates in meetings and trainings as necessary to facilitate the above duties, including those geared toward implementing evaluation tools that determine the effectiveness of Health Educator functions.
5) Completes necessary reporting and documentation associated with the above activities per organizational and regulatory requirement.
6) Performs other duties as assigned.

Description of Primary Attributes

Professional & Technical Knowledge:

1) Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, technical or health care related knowledge frequently acquired through completion of a trade school, para-professional, certificate type program, or equivalent work experience.
2) Bachelor’s Degree in related field preferred.

Technical Skills:

1) Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs and other elements.
2) Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.
3) Ability to create basic presentations in Microsoft PowerPoint.

Licenses & Certifications: None required.

Other Requirement: As a condition of employment at Family HealthCare Network, all employees assigned to work with the Cutler Orosi School District must pass a Federal Background Check.

Communications Skills:

1) Job duties require the compilation of information prepared in effective written form, including correspondence, reports, articles or other documentation.
2) Effectively conveys technical information to non-technical audiences.

Pay Scale:

Min Hourly Rate: $17.69
Max Hourly Rate: $22.99
