POSITION SUMMARY

To work with BHA associates, the community and our adult and pediatric patients providing health and nutrition education to improve quality outcomes in care, in patients with multiple diagnosis. Providing collaborative, comprehensive, and person-centered care and education conducive to behavior change and improved quality of life. Supports and advocates for patients to optimize quality care and is an expert health and nutrition resource for people and providers. Addresses the nutritional issues that arise from medical conditions, including surgical support, weight concerns, dietary guidelines for food allergies and intolerances, and lifestyle change support related to nutrition and diet. Promoting self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

GENERAL EDUCATION REQUIREMENTS

  • Bachelor's degree in Health Education or related field; Advanced degree preferred
  • Minimum 5 years of prior outpatient/clinical experience in health education role preferred

ADDITIONAL QUALIFICATIONS

  • Current Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
  • General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR).
  • Demonstrates strong oral and written communication skills

PRIMARY ACCOUNTABILITIES

  • Provides quality and effective health and nutrition education in individual and group settings to those with multiple diagnosis including prevention and active disease management.
  • Exercise sound judgment and developed clinical skills to evaluate, interpret, monitor, and document the educational needs of patients and implement the patient's plan of care based on the diagnosis in a timely manner.
  • Gains confidence and cooperation with the patient, their family/support group, and/or other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication and establishes mutual learning goals and behavior change goals.
  • Collaborates and confers with services and associates internal and external to BHA to effectively meet the health promotion/disease prevention, acute, chronic, and long-term needs, based on the plan of care.
  • Implements best practices throughout the care of a patient evaluating and re-evaluating progress and adjusting health and nutrition plans of care accordingly.
  • Utilize appropriate teaching techniques that are sensitive to the person's learning preferences and both written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
  • Maintain knowledge of current developments, trends and techniques in chronic disease management and education and makes recommendations regarding use in patient program.
  • Develop, review, update and implement educational materials to meet the needs of other professionals and the target population of patients.
  • Serve as a resource person and mentor to staff and community related to clinical knowledge of nutritional management.
  • Travel within different BHA clinics and will reimburse your local travel, per BHA policy and the current IRS reimbursement rate.
  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings to achieve departmental and corporate objectives.
  • Protects and respects patients’ rights and confidentiality ensure the privacy of each patient’s protected health information (PHI).
  • Adheres to all local/state/federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
  • Takes ownership and represents Brevard Health Alliance’s mission and organizational goals established by the executive committee. These include but are not limited to Productivity, Quality, Patient Satisfaction, and Employee Retention. Each BHA employee must incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s performance plan to be evaluated annually.
