Position Summary
The HIV and PrEP Coordinator plays a key role engaging, retaining and monitoring the clinical care for persons living with HIV/AIDs (PLWHA) and for patients taking PrEP for HIV prevention. The Coordinator works closely with the HIV Multidisciplinary Care Team. The HIV and PrEP Coordinator will provide oversight of LCHC’s HIV and PrEP patient panels to ensure that patients in these two groups are appropriately engaged in treatment to prevent severe illness and reduce public health risks to LCHC’s broader community, in concert with the Clinical Champion for HIV and the Clinical Complex Care Director.
Principal Duties and Responsibilities
Program Coordination and Panel Management

  • Utilize Analytics Reports and EMR tools to effectively manage LCHC’s HIV and PrEP patient panels and ensure timely follow-up to improve clinical outcomes. This includes ongoing monitoring of the HIV, PrEP, and Long-Acting Injectable Panel reports, Tickler Report, and other tools to ensure comprehensive and timely provision of services. Complete data-entry and chart review to ensure accurate information as needed.
  • Work closely with LCHC’s Care Managers responsible for supporting PLWHA, alerting care-team based Care Managers on identified care gaps, and keeping the Care Management Director apprised of any trends.
  • Coordinate New Patient Disclosure Visits with LCHC PCPs and members of the HIV Multidisciplinary team, to ensure coordination of care and rapid start of treatment.
  • Participate in planning and facilitation of LCHC’s Consumer Advisory Board for the HIV program.
  • Assist with document preparation and other program tasks to prepare for HIV Program site visits, audits, and other reviews.
  • Work with the Clinical Quality and Complex Care teams to identify additional priority populations to develop and monitor panels which may include Hepatitis C and other Infectious Diseases.

Patient Engagement

  • Conduct direct patient outreach to PLWHA who are not enrolled in LCHC’s Care Management services who are overdue for key services based on review of Program Panel data. Provide direct assistance to these patients related to medication access, appointment engagement, external referrals, insurance enrollment and assistance programs, and communication with family and other patient supports. Outreach may include Home Visits in certain circumstances.
  • Participate in engagement and outreach in the community, shelter, and clinic settings to promote awareness of HIV/AIDs testing and care, PrEP services, and other Infectious Disease priorities.
  • Directly manage a panel of patients using long-acting injectable medications to ensure patients have consistent access and appointments to stay up to date on their injections, in collaboration with the HIV Pharmacist.
  • Outreach to PrEP-eligible patients to offer education and engagement on PrEP and schedule follow up visits with clinical providers for interested and eligible patients. Support LCHC’s efforts to grow, improve, and engage its PrEP services.
  • During Open Enrollment period, work alongside LCHC’s Enrollment Team to assist with ADAP applications and documentation as needed to ensure PLWHA have appropriate medication and insurance coverage.

Professional Development & Accountability

  • Serve as a resource to assist LCHC staff and teams engaging with persons living with HIV/AIDS and other infectious diseases through training and consultation.
  • Participate in regular supervision and staff meetings, presenting cases or discussing work as requested.
  • Complete required or recommended training as requested by supervisor and/or LCHC leadership.
  • Complete timely and accurate documentation in the Electronic Medical Record (EMR) of all activities.
  • Perform other duties as required to assist LCHC in achieving its mission.

  • Excellent written and verbal communication skills for communicating coherently and professionally with patients and co-workers.
  • Flexibility to work at different LCHC sites and on weekends or evenings, if necessary.
  • Must be familiar with laws and mandates related to confidentiality, including HIPAA regulations.
  • Willingness to learn and practice a Harm Reduction model of care and de-escalation skills.

Required Knowledge, Experience, or Licensure/Registration

  • Either 3+ years of relevant work experience in Care Coordination, or Outreach to PLWHA or Bachelor’s Degree from an Accredited University strongly preferred.
  • Healthcare experience preferred.
  • Experience serving and working directly with the care for PLWHA strongly preferred.
  • Bilingual in English and Spanish preferred.
