Description
- Provides personal care and assists with home care patients, under the supervision of the RN Case Manager or therapist.
- Initiates and maintains positive relationships with patients customers and co workers.
- Observes and reports patient status changes including skin and psychosocial, takes responsibility for self development and supports a learning environment, displays commitment to the mission of UHHS UHHCS and its values.
- Provides personal care and companionship as directed by plan of care.
- Provides light housekeeping and light meal prep tasks as directed by plan of care, effectively communicates both verbally and in writing.
- Successful completion of agency written test and competency evaluation of skills in the field within one month of hire and annually.
- This position is in Home Care Services.
- Day shift-40 hours per week.
- Skilled position.
- The territory is Ashland County.
Qualifications
- High School Equivalent / GED
- STNA - State Tested Nursing Assistant
- Valid drivers license required and car insurance coverage.
- Minimum one year home care experience preferred.
- Completes required continuing education requirements to maintain certification.
- Current CPR certification or obtain within one month of hire.
State Tested (STNA) or have 75 hours of Aide classroom training, 16 hours of which is supervised practicum training required.
