Medical Social Worker (MSW)
to provide optimum care to our patients in
San Joaquin County (Stockton, Lodi, Manteca, Tracy).
This position is a Pay per visit compensation model.
Pay per visit ranges from $80- $100/ depending on type of visit.
Home health experience a plus!
Spanish speaking a plus!!

What You Will Be Doing:
Job Summary
Major responsibilities include assessing and evaluating patients for their need of community resources and in home support services, assisting in the implementation of the plan of care, assisting patients with advance directives, and preparing and submitting accurate clinical notes. Our visit notes and medical records are 100% electronic allowing you to complete all of your charting inside the patient’s home.
To be considered, all applicants must be licensed in the state of California by the Board of Examiners. One year of social work experience in a health care setting required.
Responsibilities

• Completes competency checklist and joint patient visit with supervisor or therapy preceptor.
• Assisting the physician and other team members in understanding the significant social and emotional factors related to the patient health problems.
• Participating in the development of the plan of treatment or Plan of Care (POC).
• Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
• Advising, counseling, and when appropriate, instructing family in patient’s social needs.
• Regularly re-evaluates medical home equipment.
• Utilizing appropriate community resources.
• Participating in discharge planning.
• Participating in in-service education programs.

Qualifications

• Master Degree from a program accredited by the Council on Social Work Education required.
• Certified/licensed and in good standing within the state currently working in according to clinical type. LCSW License is highly preferred
• One (1) year experience as a MSW. One year of experience as an MSW in a health care setting is strongly preferred. Home Health experience is preferred.
• Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current Basic Life Support Certification - BLS required.
• Is flexible and cooperative in fulfilling the role obligations.
• Demonstrates tact, patience, and good personal hygiene.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client related activity practices.
