Overview:
Provide admission, case management, and follow-up skilled psychiatric nursing visits for home health patients.
Provide on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Supervisor.
Conference with physician to develop the initial plan of care and have continual dialogue to update the plan of care as necessary.
Coordinate with other disciplines of care such as Physical Therapists, Occupational Therapists, MSW, Home Health Aides and external providers.
Participate in case conference with interdisciplinary team to appropriately case manage patients.
Participating in the Quality Assurance/Quality Improvement Process.
Adhering to all practice standards as they apply to patient care.
Such other tasks as necessary to provide appropriate care for the patients. Qualifications:
RN with a bachelor’s or Associates degree in nursing and must have two years of recent nursing experience within the last 3 years in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program, or other outpatient psychiatric services.
Required Experience
Minimum 1 year acute care med/surg nursing experience preferred.
Kare-In-Home is looking for a
Psychiatric Registered Nurse to provide services to its home health patients in
Jackson County.
Offering $32 to $48 depending on services rendered.
Responsibilities:
Psychiatric RN will be expected to perform the following:
