CareOne Home Health is looking for Registered Nurses for our growing Home Health business serving Calaveras, Tuolumne and Amador Counties. Mileage is reimbursed and driving time is paid. Fulltime or part-time positions are available.

The Registered Nurse identifies the patient's unique health needs and priorities, establishes an individualized plan of care and therapeutic relationship with the patient, and communicates the plan to other members of the team.

Job Responsibilities Include

1. Assumes responsibility of assigned patient’s care from start of care through discharge
2. Understands the patient’s reimbursement coverage and requirements for ongoing care eligibility
3. Assesses patient and caregiver needs at OASIS time points and throughout the episode
4. Develops, communicates and coordinates the plan of care with other disciplines
5. Coordinates the patient visit schedule in collaboration with other Pod members
6. Schedules own visits and ensures Visit Clinicians schedules are in system
7. Plans and leads weekly Team/Pod update
8. Attends and prepares for case conference with Clinical Director/Manager and completes the case conference documentation
9. Provides patient and caregiver education including but not limited to protocols and disease management programs (CHF, COPD, DM, Medication Management, etc.).
10. Manages internal and community resources needed to implement the plan of care
11. Communicates/collaborate with patient, family, MD, other Team/Pod members, and community resources.
12. Evaluates and follows up on all pertinent aspects of care (lab work, medication changes, sudden changes in status etc.)
13. Continued monitoring, reassessment and re-evaluation throughout the episode
14. Participates in discharge planning and scheduling in collaboration with Pod members
15. Evaluates adherence to care plan including visit frequency, treatments, interventions and goals
16. Obtains and verifies Resumption of Care (post hospital) orders either directly and/or collaborates with Visit Clinicians
17. Ensures recertification is timely and appropriate
18. Ensures medication reconciliation and management is completed
19. Ensures wound care is provided per the plan of care
20. Makes changes to the care plan as needed and initiates verbal orders and/or collaborates with the Visit Clinicians
21. Home Health Aide (HHA) supervision
22. Coordination of care communication
23. Discharge planning
24. Documentation

Requirements:

• Minimum of 1 year of clinical experience
• Current CA Registered Nurse license
• Valid driver's license and required automobile insurance
• Prior experience in home health, hospice, infusion, or home care is preferred
• Experience with electronic documentation is preferred
