Job Description:

The Homeless Support Case Manager is responsible for providing case management, resources and support to unhoused people in the community. This is in collaboration with the County of San Luis Obispo’s Countywide Plan to Address Homelessness specifically to unhoused individuals participating in the Oklahoma Safe Parking operation and other specific encampments as expressed by the County Homeless Services Division.

Under direction of the Housing and Homeless Services Director, and in conjunction with fellow providers, the Homeless Support Case Manager will engage unhoused members of the community in an effort to connect people to services and housing. The Case Manager will provide services with a trauma informed philosophy and develop rapport and relationship with clients in order to help access food, clothing, housing, behavioral health services, and assist them in maintaining their services once they are secured. The Case Manager will follow through with referrals, track client progress in securing and maintaining resources, and communicate with community service providers in order to support clients in the program.


Hours/ Salary

Full-time, 40 hours per week. Anticipated hiring range is $20.00-$24.00 per hour depending on education and experience. Includes a generous individual benefits package.

Experience and Skills:

BA/BS AND 6 months of experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of experience with target population, OR 4 years of full time experience with target population of unhoused community members. Experience working as part of a collaborative team delivering in-the-field services and supports to people with mental health and/or substance use issues is PREFERRED. Bilingual (English/Spanish) is PREFERRED. Working knowledge of San Luis Obispo County protocols regarding assessment, diagnosis, prognosis, counseling, and methods of developing and implementing a treatment plan; human service delivery system; current trends in the field of mental health and Trauma Informed Care; and laws and regulations applicable to mental health. Ability to link and support people to effective counseling and psychotherapy with individuals, families, and groups; develop and maintain cooperative relationships with clients’ families, members of other professional disciplines, social service agencies, and co-workers; relate professionally with persons with serious mental disorders; identify and utilize community resources; prepare and present clear and concise oral and written reports. Must have experience working with persons with severe and persistent mental illness. Must have experience working with persons who are chronically homeless, unhoused, or at risk of becoming homeless. Strong interpersonal skills, collaborative work style and oral/written communication skills. Well defined organizational and time management skills. Ability to work independently and flexibly. Able to deal effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level. Ability to handle intense situations, display good judgment and decision making skills, and remain calm and clearheaded under pressure. Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices. Conduct self in such a manner as to assure at all times, maximum safety to one’s self, fellow employees, clients, and other persons contacted through TMHA business. Conditions of employment include: Must be at least 18 years of age; possess a valid CA Driver’s License, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance. Ability to possess and maintain good physical and mental health. Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting. Must be able to successfully complete First Aid and CPR training.
