Hospice LPN Case Manager Job at Lakeview Christian Home of the Southwest
Provide direct and indirect client care services primarily delivered on a visiting and intermittent basis in the client’s home and in accordance with the policies and procedures of Lakeview Christian Home Health Care. Makes the initial evaluation visit, regularly re-evaluates the client’s nursing needs, prepares clinical and progress notes, coordinated services. Must have a valid New Mexico LPN Nursing license, driver's license, have automobile insurance and be able to pass a background check.
