VCU Health at Home by BAYADA is looking for compassionate and dedicated full time Hospital at Home Registered Nurse to join our team in our Richmond, VA office. for night shift. This is a Full Time Opportunity!

Visits are provided over a 12 hour / shift, and you can schedule what nights you are available!

A Hospital at Home RN will ensure quality care during daily in-person visits to patients receiving acute care at home through Hospital at Home. This includes the ability to identify patient's needs through acute care and home assessments then implementing and monitoring the medical treatment plan in collaboration with the 24/7 virtual nursing team and providers.

Registered Nurse (RN) Responsibilities:

• Ensure quality patient care across the continuum, reduce avoidable readmissions, and improve outcomes for patients enrolled in the Hospital at Home program.
• Provide quality care and timely response during urgent in-person assessments when requested by the Hospital at Home virtual nursing and provider team. Diagnostics and treatments within the scope of nursing may be requested by the team and communication back to the team regarding the assessment, results of diagnostics, and response to treatment will be crucial.
• Ensure individuals are receiving appropriate services during their Hospital at Home stay including but not limited to therapy, food, consultative, mobile imaging, labs, transportation, medication administration, and medical equipment.
• Provide high level nursing care through the ability to perform phone triage and intervene or escalate appropriately using critical thinking skills.
• Ensure patients and/or caregivers are confident and comfortable in self-care management through use of teach back education strategies.
• Ensure appropriate screening of patients in need of continuum integration services prior to hospital discharge including but not limited to transitional remote patient monitoring and telehealth virtual visits, and Home Health or Home Hospice care transitions.
• Provide direct bedside patient education and consultation on continuum integration services as needed.
• Provide community resources appropriately in order to prevent un-necessary hospital readmissions by collaborating with the interdisciplinary care team such as social work, virtual nursing team and care coordination assistants, and providers.

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in Virginia
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
