POSITION SUMMARY: To provide support to individuals applying for and/or receiving housing subsidies including but not limited to, RAP, Next Steps, HUD, Pilots, and SHP, by assisting with linkage to support services throughout the Valley and Shoreline with the goal of ending homelessness and prevention of returning to homelessness.

  • Assist individuals with the application and eligibility process for designated supportive housing programs funded through HUD, DMHAS, DOH, and other funding sources. Work with applicants to complete application for housing assistance, including obtaining documents and identifications.
  • Assist in determining basic eligibility of supportive housing programs applicants as dictated by current HUD, DOH and Coordinated Access Network definitions and policies.
  • Assist homeless individuals with all aspects of housing search and placement in accordance with BHcare supportive housing programs including working with clients to devise an appropriate housing search strategy if applicable.
  • Negotiating with landlords and realtors as needed. Advocate for individuals as indicated
  • Support program participants in making the transition from homelessness to permanent housing by assisting them in maintaining the terms of their lease agreement, program guidelines and individual service plans.
  • Complete housing inspections, reassessments, acuity index every 6 months. Complete annual reassessment including HQS full inspection, acuity index, rent reasonableness, rent recertification, household eligibility and required releases of information and signature notifications.

QUALIFICATIONS:

  • Bachelor's degree and a minimum of 2 years human services experience.
  • Experience working with homeless population preferred.
  • Ability to work with diverse populations.
  • Must have working knowledge of computer applications, and if applicable ability to navigate through electronic health record
  • Active and valid CT driver's license and vehicle
