Position Summary:
In order to allow for timely completion of the health record, the HPF Chart Analyst/Birth Certificate Tech will review discharged patient’s record for incomplete or missing items, identify the responsible practitioner for each of these items, and link the deficiencies to the appropriate physician for completion. The HPF Chart Analyst/Birth Certificate Tech will be responsible for verifying chart completion. Responsible for completing birth certificates within the State of Texas requirements and transmitting the information to the Bureau of Vital Statistics in Austin, Texas. Responsible for providing the Acknowledgement of Paternity (AOP) for patients not married to each other. Must be certified with the Attorney General’s Office and maintain that certification.
Education:
High school diploma or equivalent.
Training and Experience:
Experience in a general office. This position requires the ability to retrieve, communicate or otherwise present information in written, auditory, or visual fashion is essential. The methods used to express or exchange ideas are by spoken or written word. Written telephone and manual dexterity skills are also required. Must be able to work unsupervised, be self-motivated, with excellent customer service skills. Will be expected to achieve Certification for Acknowledgement of Paternity with the Attorney General’s Office.
Job Knowledge:
Knowledge of the Medical Staff Bylaws in regard to chart completion. Knowledge of Privacy laws both for the State of Texas and HIPAA. Must be able to type 40 words per minute with extensive PC knowledge and skills that are verifiable through testing or previous job experience. Knowledge of the law on Acknowledgment of Paternity (AOP) with the Attorney General’s Office. Bilingual preferred.
