The Dragonfly program is specifically designed to work with children and families currently involved with Child Welfare Services. JOB SUMMARY

The Intake Coordinator reports to the Program Director and is responsible for processing all referrals related to Dragonfly’s services. These services include completing all new admissions in a timely manner with the goal of opening new clients for mental health services.

ESSENTIAL JOB RESPONSIBILITIES

  • Ensure all new referrals are logged and entered into the Electronic Medical Record (EMR) in a timely manner.
  • Ability to organize and prioritize work so as to perform assigned duties independently.
  • Determine and confirm eligibility of all new intakes prior to initial appointment.
  • Meet directly with parents/guardians to complete initial admission documentation and enter new episodes in the EMR.
  • Maintain integrity of the clinical information entered into the EMR.
  • Work with intra/interagency providers to ensure all transfers are linked for services, including follow up appointments prior to discharge from treatment.
  • Interacts with program staff to schedule intake assessments.
  • Interacts with community partners to receive and make referrals, including collaboration to ensure follow-up and or other resolution.
  • Ability to respond to crisis related calls, triage, and determine appropriate level of support.
  • Ensure all logs and required reports are maintained and updated.
  • Provide verbal and written reports.
  • Assist with answering phone calls or provide additional coverage
  • Ability to provide excellent customer service when handling all phone calls.
  • Participate in Community Outreach opportunities in an effort to sustain referrals for services.
  • Think and act quickly and efficiently in emergencies.
  • Comply with all Turning Point Safety policies and procedures, including but not limited to: workplace safety, reporting work related injuries, Infection Control and preventing potential safety risks for staff, clients and others.
  • Punctual and regular attendance is an essential responsibility of each employee at Turning Point. Employees are expected to report to work as scheduled, on time and prepared to start working. Employees also are expected to remain at work for their entire work schedule. Late arrival, early departure or other absences from scheduled hours are disruptive and must be avoided.

ESSENTIAL JOB REQUIREMENTS

  • Must have a high school diploma or equivalent of two (2) or more years working at a job site as a clerical and/or social services staff.
  • Experience with data entry including direct services.
  • Prior experience and comprehension of working with an EMR Database preferred.
  • Ability to work well with a diverse group of people.
  • Maintain all trainings related to current job duties.
  • Communicate effectively in written and spoken English.
  • Ability to communicate effectively in written and spoken Spanish preferred.
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment physical, drug screen, general physical, and TB test.
  • COVID19 vaccination required prior to employment.
  • Must maintain good relationships with current, future and past employees, co-workers, vendors, third party administrators, and others with whom the Agency is transacting business, and relate to them in a professional manner.
