Summary/Objective
Revival Home Health Care is seeking an Intake RN/Marketing Liaison, who will be responsible for managing relationships with facilities, hospitals, and patient families while working collaboratively as a team member to provide excellent customer and patient experience. This position also serves as an on-site resource to implement discharge plans for inpatients and their families. This position requires an individual who is highly motivated, result driven. The candidate must have excellent oral and written communication skills in order to interact effectively and professionally with potential referral sources and agency staff.
Essential Duties and Responsibilities
- Reviews patient information sent to the agency to determine clinical appropriateness for admission
- Participates in coordination of home health services, appropriately identifying needs for home health disciplines (SN, HHA, OT, PT, MSW, ST)
- Collects required documentation from facilities to ensure clinical compliance with regulatory requirements
- Promotes and fosters ongoing collaboration between healthcare professionals and Revival Home Health Care while on-site the facility
- Facilitates communication between patients, families, and Revival Home Health Care
- Builds and maintains relationships with referral sources
- Functions as part of a collaborative team, escalating concerns as needed to clinical supervisor
- Able to work under moderate supervision
