Summary
:

Perform functions to prepare, support, and train clients for employment in the community. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Essential Responsibilities, include but are not limited to:

  • Members to complete work assignments per employer specifications
  • Monitor and report member progress based on established outcomes in a timely and efficient manner.
  • Assist employers in communicating with the members to facilitate member/employer relations.
  • Complete accurate daily time sheets, monthly case notes, and reports for all members in your caseload and route these documents as necessary to IDT members and in agency clinical documentation system.
  • Accurately report billing for services on a monthly basis in clinical documentation system according to DHS settings rule standards.
  • Communicate service and active treatment needs of members to the Job Placement Director.
  • Participate in member staffing.
  • Conduct job trial evaluations for new members.
  • Perform job development activities that offer current site expansion or the development of new sites.
  • Responsible for agency vehicle operation and transporting work crews.
  • Ensure safe working environment and practices for members at work sites, including review of fire and emergency procedures to assure prompt and safe responses.
  • Write and file member progress notes and Title XX documentation on proper forms.
  • Participate in regular staffing reviews and follow through with recommendations
  • Respond to medical and behavioral emergencies following appropriate health & safety, crisis prevention, first aid, CPR, accident/incident reporting and other procedures, as necessary.
  • Attend to total body care and appropriate dress hygiene (such as hand washing), safety and medical needs of members.
  • Attend mandated training within prescribed time periods and attend other in-services, as requested.
  • Support members in self advocacy activities
  • Attend regularly scheduled staff meetings.
  • Participate on agency committees.
  • Distribute members checks when appropriate.
  • Assist members in placement at job sites.
  • Perform other related duties as assigned.
  • Supervise supported sites with members on staff.
  • Serve as a liaison between employers and members in their workplace and exhibit professional and problem-solving skills in order to assist member sand obtain employment.
  • Assist with job searching by helping with applications and the onboarding process for the members.
  • Identify job and career advancement opportunities based off needs, objectives and skill set.
  • Prepare members for job interviews.
  • Liaise with employers to identify possible job opportunities for members with disabilities.
  • Help in the development of motivation and skills.
  • Communicate with members to understand their goals and ambitions.
  • Accurately track members work schedules.
  • Must have the ability to travel to different employers to conduct job site visits to assess work performance.
  • Manage caseloads of assigned members and ensure retention of employment.

Requirements:

QUALIFICATIONS

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

EDUCATION, SKILLS, and/or EXPERIENCE

  • High school diploma; Bachelor's degree preferred.
  • Minimum of one year work experience in rehabilitation, social work, special education, or related field.
  • Knowledge of business and industry practices helpful.
  • Must have a valid driver's license.
  • Ability to travel locally per the needs of the agency.
