We’re seeking a competent, compassionate, and career-oriented professional to serve as our Lead Care Coordinator - Wraparound .

Job Purpose:

Our next Lead Care Coordinator – Wraparound will be responsible for coaching, modeling, and accompanying uncertified Care Coordinators to acclimate to their role. Our next Lead Care Coordinator will provide crisis planning and connection to mental health resources for family stability and/or placement in a primary caregiver’s home

Duties:

  • Demonstrates individualized and creative planning for families that is aligned with the beliefs, values, and culture of the family
  • Effectively facilitates the Child and Family Team Process, including the engagement of informal and natural supports.
  • Provides Care Coordination services in a manner that exemplifies the values and mission of Wellpoint and WAM
  • Demonstrates ability to use a strength-based and community centered approach in working with the child/family
  • Holds monthly Team/POC meetings for each enrolled family.
  • Maintains progress note documentation timeliness
  • Completes all necessary paperwork timely and in a strength-based and quality manner per WAM and Wellpoint requirements, including court letters, change of placement forms, SARs, referrals, POCs, progress notes, evaluation instruments, consent forms, etc.
  • Assists supervisor with providing clinical direction, consultation, crisis intervention, and conflict resolution for care coordinators and Child & Family Team (CFT) members that support family centered and strength-based service delivery
  • Role models and coaches all staff on the Values and Principles of the Wraparound Process.

Qualifications:

  • BA or BS Degree in Social Work, Psychology, Nursing, Occupational Therapy or BA or BS in unrelated field with experience in human services, MS/MSW preferred.
  • Experience with project management, information tracking and management and operational protocol preferred.
  • Basic understanding of family system, experienced in working with emotionally disturbed children and their families.
  • Knowledge of AODA issues, sexual abuse issues and sexual abuse issues.
  • Ability to manage caseload and deal with the courts and public-school systems.
  • Ability to maintain confidentiality when reading, typing, or hearing matters pertaining to clients.
  • Ability to walk, sit, bend, and lift up to 30 lbs.
  • Must be honest, dependable, and able to meet deadlines; Initiative-taking and able to work independently.
  • Ability to demonstrate critical thinking skills, detail oriented, highly effective verbal, and written communication skills.
  • Demonstrate an ability to effective understanding and practice of cultural humility and ability work and communicate respectfully with individuals from diverse backgrounds.
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and databases, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Valid Wisconsin driver’s license required.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.

  • Regular travel in personal vehicles is required with the ability to move intermittently throughout the workday.
  • Fast paced, flexible work schedule includes evenings and some weekends to accommodate program need.
