Responsibilities:

• Supervises facility floor staff when needed
• Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client's needs and concerns
• Consults with the Clinical Director regarding the supervision of services
• Provide medication administration and wound care in a facility setting

Qualifications:

• LPN State License – DC
• Current and valid CPR/ BLS Certification
• TB vaccination, Fully COVID Vaccinated and MUST BE BOOSTED and Hep B vaccination
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• Clear criminal background and driving record
• Two professional references within the health care field
• Prior home care experience is strongly preferred
• Self-starter with the ability to work independently and manage time effectively
• Strong interpersonal skills, including a willingness to teach and mentor caregivers
• Comfort with technology; prior experience with electronic medical records preferred
• Ability to drive and travel to cases as assigned
• Availability for weekdays and some weekends
