Qualifications of a Registered Nurse (RN):

Position Summary:

Under the direction of the Registered Nurse, assists in assessment, planning, implementation and evaluation of care of resident within the framework of the nursing process. Works within the philosophy, purposes, policies and standards of both the Facility and Nursing Department. The incumbent performs nursing procedures for which preparation as Licensed Practical Nurse has provided the necessary knowledge, skills and abilities.

Qualifications:

  • Individual must be able to service residents with a wide range of disabilities, including those with critical and complex diagnosis.
  • Must be able to read, write, and speak English.
  • Must meet sensory requirements of speech, touch, vision, and hearing to normal with corrections as applicable.
  • Must have effective communication skills, both verbal and written.
  • Must be creative effective and have good observation and analytical skills.

EDUCATION:

  • Graduate of an accredited Licensed Practical Nurse Program.

PHYSICAL DEMANDS:

  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must function independently, have personal integrity, have flexibility, and the ability to work effectively with other personnel.
  • Must meet the general health requirements west forth by the policies of this facility to work effectively with other personnel.
  • Must be able to push, pull, and move things.
  • Must be necessary to assist in the evacuation of residents during emergency situations.

RESPONSIBILITIES:

  • Accepts responsibility for own practice.
  • Performs nursing care in relatively structured, routine situations. Administers multiple forms/methods of treatment, under the direction of the RN Supervisor
  • Prepares residents for diagnostic tests, treatments, examination, PT/OT, etc., according to protocols.
  • Sets- up required documentation for physician visits and transcribes orders timely.
  • Prepares and administers medications, according to protocols.
  • Follow facility procedures /protocols for admitting residents, transferring residents to hospitals/clinics/outpatient and completion of appropriate documentation based on established guidelines.
  • Administers intravenous infusion in accordance with the facility’s Nursing Department Policies, procedures and Infection Control Guidelines.
  • Reports pertinent observations, treatments, medications, incidents and accidents associated with residents, to the Registered Nurse, and records same as per Nursing Department standards for documentation.
  • Receives, reports, and counts controlled drugs with the outgoing shift. Gives report and count of controlled drugs to the incoming shift.
  • Makes rounds on all resident rooms, and ensure that the Nursing Assistants under her supervision receive their resident reports, has their assignments and break time scheduled.
  • Responsible for daily monitoring of Electronic Medical Record’s dashboard (including “alerts”, incomplete UDAs, Immunization entries, Medication/Treatment alerts, etc.) and addressing any issues identified.
  • Ensures new admissions, discharges and transfers are entered in the Electronic Medical Record (Quick ADT) as soon as it occurred.
  • Contributes to, and updates nursing care plans in collaboration with other members of the professional health care team.
  • Reinforces resident/significant others, teaching programs.
  • Assist with rehabilitations programs of resident according to plan of care.
  • Provides for physical comfort and safety of residents.
  • Contributes to resident care conferences.
  • Participates in physician rounds and the discussion of residents’ problems, progress and plans.
  • Assists in emergency resuscitative procedures by initiating a “Code Stat” call, performing resuscitative procedures and carrying out other such activities as directed.
  • Participates in unit-based rehabilitation nursing educational conferences as well as facility wide In-service.
  • Collaborates with nursing staff in establishing educational plans for residents/families.
  • Participates in planning/organizing and evaluating resident care priorities and objectives.
  • Maintains documentation according to the Nursing Departmental Policies and Procedures Manual.
  • Documents all information in a complete, concise, accurate and timely manner.
  • Respects the confidentiality aspect of all resident records.
  • Demonstrates age-specific cognitive, psychomotor and psychosocial competency expectations distinct to assigned clinical area that addresses residents’ (particularly frail and elderly) physical, psychosocial and cultural needs.
  • Reports pertinent information to the Primary RN and ACC/Supervisor on a timely basis.
  • Interacts with resident/family, utilizing communication skills appropriate to the situation.
  • Receives discusses and communicates information to other staff members in clear and professional manner.
  • Is sensitive to resident needs and responds accordingly.
  • Seeks additional duties when assigned.
  • Identifies and actively pursues a planned program for professional growth through continuing education, reading journals, etc.
  • Demonstrates ability to recognize and attend to priorities.
  • Adapts readily to changes in time requirements and workflow.
  • Works effectively and at a steady pace.
  • Provides proper notification of absences or tardiness, in accordance with Facility Policy.
  • Maintains good attendance, with minimal absences.
  • Washes hands before and after each resident care procedure.
  • Wears gloves during venipuncture.
  • Adheres to Facility and departmental guidelines concerning Safety, rules of conduct, HIPAA, etc.
  • Maintains a professional appearance wears an identification badge at all times when in the Facility.
  • Respects the patient’s rights to confidentiality at all times.
  • Displays sensitivity to the needs of patients, visitors, and co-workers.
  • Greets and interacts with others with consideration, courtesy, and respect.
  • Communicates effectively with patients, visitors, and co-workers, in a clear, timely, and professional manner.
  • Performs work duties willingly and with initiative.
  • Remains calm, and continues to work effectively in stressful situations.
  • Displays friendly, helpful behavior to patients, visitors, and co-workers. (Good customer service).
  • Uses professional judgment and flexibility in presenting alternatives or innovative solutions to problems encountered.
  • Cooperates with other Departments or work groups/peers.
  • Demonstrates appearance and hygiene appropriate to the delivery of quality services. Maintains professional appearance adhering to the state uniform policy including identification badge.
  • Complies with fire, disaster, infection control, safety hazard, and emergency procedures.
  • Reports to work on time and is ready work at start of shift; adheres to department schedules.
  • Give appropriate notice of absenteeism and does not abuse time off benefit.
  • Practices in compliance with the Nurse Practice Act and regulatory agencies’

  • Assures that a stock level of medications, medical supplies, equipment, etc are maintained at adequate levels at all times.
  • Assure that nursing personnel, to include agency/registry nurses, follow established nursing procedures.
  • Assure that the total needs of residents are met by assuring that sufficient number of nursing personnel are on duty at all times.
  • Assume the authority, responsibility and accountability of directing your nursing shift. Be familiar with the Charge Nurse’s responsibilities for the assigned shift.
  • Responsible for ensuring that all EMR Immunization entries, MAR/TAR entries and other required documentation are completed on time.

  • Assures that the resident's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are followed.

  • Observes rules, policies and regulations with regard to attendance, punctuality, appearance and professional attitude as established by the facility.

Miscellaneous:

  • Conforms to the HIPAA Compliance Program and applicable facility policies for patient privacy.
  • Conforms to the facility’s Corporate Compliance Program to include Code of Conduct and Conflict of Interest
  • Conforms to the facility’s policies and procedures.

Physical:

  • Able to stand times (2) plus hours without interruptions or breaks.
  • Able to push, pull, move, and or lift a minimum of 20 pounds to a minimum of 4 feet or carry 20 lbs. a minimum of 2 feet.
  • Must be able to sit, stand, bend, lift and move intermittently throughout the day.
  • Must be able to assist with the evacuation of residents during emergency situations.
  • Must pass Medical and Physical examination based on job qualifications, requirements and duties/responsibilities.
