Salary Range - $105,000.00 - $115,000.00

The Manager, Health Home is responsible for managing the activities of the Care Management Health Home Team and the performance of each staff member. The Manager provides direct care as well as oversees the staff to ensure coordination of members’ overall care which includes a range of health and social supports aimed at maintaining health, mobility and independence in the community. This work is carried out in support of the mission and goals of Independence Care System.

Essential functions

  • Able to demonstrate excellence as a RN and provide member centered disability-competent care which is reflective in a dynamic plan of care (POC). Will perform Health Home Comprehensive assessments as needed.
  • Lead a team of 8-10 staff majority is CNs maybe an RN that have expertise in addressing common needs/barriers for people with physical disabilities (PWD). This includes: monitoring individual staff’s ability to complete assessments and provide ongoing disability-competent care coordination with members and their care team.
  • Ensures that team’s targeted meaningful billable productivity is achieved each month and clearly documented in the members’ POC.
  • Demonstrate proficiency in Motivational Interviewing (MI) and utilize it to identify barriers, strengthen personal motivation for and a commitment to a change.
  • Acts as a resource to all staff by sharing expertise and resources with a goal of increasing staff’s understanding, skillset and autonomy to provide PWD competent care. This includes the use the practice of motivational interviewing to bring about change.
  • Actively participates in CM Operation and leadership meetings and contributes to ongoing process planning and CM development, training needs.
  • Respond to the needs of members and the organization, even if those needs do not coincide with the details of this job description.

Requirements

  • Ability to meet current job requirements including developing and/or coordinating a PWD competent POC and monthly meaningful contacts requirements.
  • Highly self-motivated and able to manage time independently.
  • Direct experience in Care Management
  • Excellent interpersonal skills
  • Demonstrated ability to have a difficult conversation, sensitivity to cultural and personal diversity
  • Demonstrated ability to work with internal and external resources, including clinicians.
  • Advanced verbal and written communication skills

Preferred Qualifications

  • Bachelor’s degree in related field
  • Experience in management/leadership roles with responsibility for direct supervision of staff.
  • Experience working with people with physical disabilities.
  • RN strongly preferred
