MyCHN is seeking a full-time Family Nurse Practitioner or Physician’s Assistant to provide family medicine, chronic disease management, pediatrics, women's health including prenatal care, and medication-assisted treatment. The ideal candidate will be embedded in an integrated care clinic that follows patient-centered medical home (PCMH) guidelines and principles.

As a federally qualified health center (FQHC), MyCHN provides dental, medical, behavioral health, and pharmacy services within the Freeport, Clute, Angleton, Alvin, Pearland, League City, Galveston, Houston, and Bacliff areas.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Applies advanced specialty knowledge, clinical skills, and expertise to assess patients and develop a list of actual and potential needs/diagnoses.
  • Responsible for gathering health history of patients through interview of patient and/or family member and physical examination.
  • Identifies risk factors through discrimination of normal and abnormal findings obtained thorough health history, physical examination, laboratory and/or radiological test results, and behavioral assessment.
  • Applies advanced knowledge, skills, and expertise in planning and implementation of patients' care.
  • Must maintain patient documentation in a prompt and comprehensive manner.

REQURIEMENTS/QUALIFICATIONS:

  • Must possess a current and unrestricted license by the Texas Medical Board as a Family Nurse Practitioner or Physician Assistant.
  • Experience working in a high-volume clinic environment.
  • Maintain Board Certification.
  • Current CPR, PALS or ACLS as specialty indicated.
  • Bilingual (Spanish/English) preferred.
  • Must enjoy working in a multi-cultural environment.
  • Knowledge of professional and ethical standards of health care delivery.
  • Must be Board Certified or Eligible.
  • Preferred - Experience in the health care field (primary or acute care).
  • Preferred - Experience working with non-profit organizations.
  • Preferred - Supervisory experience.
