Medical Social Worker - Home Health Hospice Job at Horizon Home Health & Hospice
Home Health and Hospice social worker has a primary role to assess patients and family members emotional and resource needs related to the care and needs of the patient, and to help facilitate access to any needs that hinder in achieving the Care Plan goals of the patient. Needs may include community resources and planning, grief management, emotional support and finding medical coverage including through Medicaid or Medicare
Requirements:
MSW
Must be able to respond to assignments in maximum 5 days
Knowledge of community resources, Medicaid application process and placement a must
Background with elderly patients helpful
Job Type: Part-time
Schedule:
- Monday to Friday
- Weekend availability
COVID-19 considerations:

Experience:
- Medical Social Workers: 1 year (Preferred)
- Social work: 1 year (Preferred)
