We are currently hiring a Medicated Addiction Treatment (MAT) Coordinator based in Worcester MA. The Medicated Addiction Treatment (MAT) Coordinator provides support to the MAT team acting as primary source for completion of state and federal reporting requirements, including but not limited to keeping patient registries. Responsible for coordinating and facilitating patient-centered, cost effective care to patients with substance use disorders that are receiving MAT services and supporting patients in overcoming Social Determinants of Health. Actively interact with all appropriate outside agencies as needed to foster relationships to support MAT and better serve existing and potentially new patients.

Essential Functions:

• Coordinate and facilitate patient-centered and cost effective care to patients in the Medicated Addiction Treatment (MAT) program.
• Provide services included but not limited to MAT patient screenings, scheduling appointments. Provide social support navigation to MAT patients, included but not limited to benefits applications, support system navigation, referrals and complementary treatment services when needed.
• Provide direct non-clinical program support and administrative services to the MAT team.
• Communicate and collaborate with providers and all members of the multidisciplinary health care team as needed to facilitate MAT patient registries, care delivery appointments and community outreach activities.
• Ensure completion of all required paperwork for existing and new MAT patients.
• Facilitate orientation to MAT program services with new patients, as part of patient engagement within a month of intake.
• Collaborate and build relationship with community based organizations providing care for patients with substance use disorder with the ultimate goal of improving coordination of services for existing patients as well as improving our referral pipeline for new patients.
• Provide support to the MAT team around external programming, coordinating with outside partners and stakeholders around community events and health center initiatives.
• Act as primary source for supporting the MAT team around grant tracking, data entry and required reporting.
• Provide support to the MAT team in the development/update of Policies and Procedures for the MAT program

Requirements:

• BA in Public Health, Social Work or other related health and Human services advanced degree
• Minimum 2 years of experience providing direct social services or similar experience.
