Summary

SUMMARY: Under general supervision, works closely with multi disciplinary team in the field and provides support to clinical staff in community health settings throughout Alameda County. This includes scheduling and coordinating outreach locations; driving and operating mobile vehicles; maintaining required supplies and equipment; provides community outreach and health education activities, contacting clients within Alameda County, providing guidance, support, follow-up and referrals to appropriate healthcare entities.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

1. Drives and operates the mobile health van to field sites (i.e. shelters, parks and meal sites) on a scheduled basis.

2. Coordinates repair, emission checks and service activities and maintains records of vehicle maintenance, equipment and operating expenses.

3. Set up Mobile Unit to provide safe care.

4. Ensures the van is maintained/cleaned and vehicle maintenance and housekeeping logs are kept current.

5. Primarily in a mobile outreach setting within the community, acts as liaison between client, relatives, guardians, employers, physicians, and service providers on behalf of clients.

6. Administers risk assessment questionnaires.

7. Assists in determining vocational goals and abilities.

8. Assists in development of case management plans; oversees implementation of plans subject to review by professional staff.

9. Assists with coordination and delivery of preventive health care programs.

10. Attends staff conferences; represents staff in community-agency meetings.

11. Collects and delivers (if applicable) participant’s informed consent forms and/or questionnaires to designated staff.

12. Conducts intake interviews with clients and relatives to obtain information concerning medical/social history and current mental condition, following established protocols.

13. Informs clients of community services available and may contact those agencies/community-based organizations on clients’ behalf.

14. Performs community outreach and health education activities to targeted groups and/or individuals.

15. Pre-screens clients to determine health care/social service needs and communicates those needs to professional or paraprofessional personnel as necessary.

16. Provides community groups with educational information concerning health/social service programs in Alameda County.

17. Recruits volunteers to attend drug treatment programs at participating drug treatment centers.

18. Translates educational and other resource materials for specific targeted population.

19. Performs other duties as assigned.

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

MINIMUM QUALIFICATIONS:

Required Education: A.A. degree from a recognized college in health services, social science or a related field such as psychology or counseling; or completion of two years of college or university with 60 semester units or 90 quarter units from a four-year college or university with major coursework in health services, social science or a related field.

Required Experience: One year full-time experience working in a community outreach program performing duties such as intake, peer counseling, assessments, program screening, placement and referral, or one year full-time experience in the class of Eligibility Clerk, Specialist Clerk or higher with AHS services performing similar duties, or the equivalent of two years of full-time experience working in a community outreach program performing similar duties; experience in the areas of Outreach program development, implementation and evaluation.

Preferred Experience: Experience in field outreach activities; experience driving 38 foot RV.

Required Licenses/Certifications: Valid California Class C driver’s license and good driving record; BLS.
