Position Summary:

Responsible for providing clinic-based nursing services providing direct age-specific and culturally appropriate care through the use of the nursing process, based on evidenced base care, serves as a clinical resource. Compensation is dependent on skills and experience.


Your Contribution to the Team Include:

  • Follows the registered nurse standards of practice to properly assess each patient to ensure appropriate medical care is delivered in the most optimal, cost effective, and culturally sensitive manner
  • Identify and assess high-risk patients using established criteria, coordinate multi-disciplinary teams and resources to support patient care needs, provide care coordination/management for chronic medically complex or fragile patients across the life span
  • Actively participates in the team care of patients with complex chronic diseases, coordinates care of complex patients, provides preventative health.
  • Working closely with primary care teams, specialty care teams and medical providers, the per diem Nurse will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self-manage.
  • Collaborates with other health care team members regarding plan of care, utilization of clinical protocols variances in plan implementation, and expected goal/outcomes to optimize health which can include but not limited to:
  • medication reconciliation,
  • insulin management,
  • Hypertension assessment,
  • INR management, and
  • initial obstetrical pre-natal intake
  • Assess, provide recommendations and next steps for walk-in and telephone call triage patients evaluating acuteness of need and/or priority of intervention and document assessment information of all activities.
  • Provide assessment, treatment and follow up related to basic wound care for patients with a range of complex medical conditions.
  • Inform and follow up with patients related to abnormal lab test and x-ray results as needed.


Must haves to be successful:

  • Current Unencumbered Washington State Registered Nurse license required.
  • Current BLS certification
  • Certification in case management highly desired
  • Proof of immunizations and/or immunity to: COVID-19, MMR (measles, mumps and rubella), Varicella, Annual Influenza, and TB are required for all employees. Additionally, if you work in a HealthPoint clinic, Hepatitis B. and Tdap (within last 10 years) are required. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immunity prior to starting or no later than their fifth (5) business day of employment.
